The tubing has been removed from the wellbore and a plug set. The well is shut-in and cage has been set over wellhead for protection. Ohio-West Virginia Excavation is onsite setting cellars for the Great Scott #3H, Outlaw A #11H, Yost A #1H and Yost B #5H.

0192160624 2018-07-19 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection Deep Well Services Rig 4 was rigged up on the Yost B5H. I spoke with Eclipse Company man and he stated that they were in the process of installing a plunger lift system. Two 500-barrel frac tanks and one 500-barrel gas buster were being utilized. All casing strings were equipped with a pressure gauge and were being monitored. The area effected by the construction of the pad has been restored with vegetation established. The location meets preliminary restoration requirements listed under ORC 1509.072(A).

1411824977 2014-09-27 Casing String Witnessed No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #359 had drilled a 24inch diameter hole to the depth of 439 feet and ran 429 feet of 18.625 inch 87.5#/ft. J-55 casing. Halliburton was on site and washed down the last 11 feet of casing then gained circulation with a total of 200bbls fresh water then pumped 25bbls of gel, 30bbls of fresh water and cemented casing with 810 sacks of Halcem cement with 2% CaCl, weighing 15.6ppg verified by mud scales and a yield of 1.22cuft/sk. Cement was displaced with 120bbls of fresh water. Good cement returns of 20bbls to surface.

They will be finishing up in the next couple of days. I did a walk around inspection while I was on location. There are no violations at this time.

During my inspection the well was equipped with a plunger lift system with valves in the open position on the production tubing. The 13.375-inch diameter surface casing had 100 pounds per square inch, the 9.625-inch diameter intermediate casing had 1500 pounds per square inch, the 5.5-inch diameter production casing had 950 pounds per square inch, and the 2.875-inch diameter tubing had 646 pounds per square inch. The production equipment consisted of eight sand traps, eight gas production units, and three 400-barrel tanks. The produced water was being separated on pad and distributed in the 400-barrel tanks. The oil and gas were being gathered at the Eclipse Hayes CGF pad via pipeline. Identification was located at the wellhead.
