0519083679 2014-11-20 Completion Testing No Violations On the day of my inspection I received a phone call from Bruce Carpenter of Eclipse stating that there was a release of brine from the Fritz pad going to the Hayes Central Gathering Facility. Upon arrival I witnessed that the air release valve had failed resulting in the man hole filling up with fluid releasing 4bbls outside of containment. The valve was repaired and excavation contaminated soil being put into containment and will be removed and disposed of tomorrow along with straw and seeding to take place.
0547188736 2017-05-30 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a sand trap, a GP unit and 2-400 barrel purge tanks. The well was equipped with a plunger lift system with valves in the open position with a cage over the wellhead as simultaneous operations were ongoing with conductors being set on the Yanosik wells. Gauges present read 400 pounds per square inch on the tubing and 615 pounds per square inch on the casing. Fluids and gas are separated at the GP unit and then transferred to the Hayes Central Gathering Facility. Identification was present and production records were current. Eclipse will set a plug in the tubing on Friday and nipple down wellhead for upcoming drilling on pad.
0951054671 2018-06-11 Production Wells No Violations The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Pressure monitoring for all casing strings were present. The production equipment consisted of three 400 barrel tanks, nine sand traps, nine gas production units, and six indirect line heaters. The identification was located at the wellhead.

The well was equipped with Sweco wellheads and production trees with valves in the open position with 2050psi on the tubing and 2450psi on the casing. Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF.

During my inspection the well was equipped with a plunger lift system with valves in the open position. Gauges were present reading 950 pounds per square inch on the production tubing,1211 pounds per square inch on the production casing, 1400 pounds per square inch on the 9.875-inch diameter intermediate, and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of six indirect line heaters, nine sand traps, nine gas production units, and three 400-barrel tanks. Well product travels via pipeline to the Hayes Central Gathering Facility.

Fluids and gas are separated at the GP unit and then transferred to the Hayes Central Gathering Facility. Identification was present and production records were current. Further inspection revealed the mouse hole may not be plugged according to OAC 1501:9-11-11 (B). I contacted Eclipse Resources and requested that the plates be removed from the mouse hole and checked for cement. If cement was not at 30 inches, they need to be cemented to be compliant with OAC 1501:9-11-11 (B).
