Once levels are confirmed replacing soil will begin with seeded to follow. Sun Pro was beginning to excavate the contaminated area on the well pad at 6-8 inch increments with testing in between. Once levels are determined acceptable ODNR will be notified and replacement of stone will take place.

The well was equipped with a plunger lift system with valves in the open position. Gauges present read 520 pounds per square inch on the tubing and 650 pounds per square inch on the casing with annular pressure monitoring in place. Identification was present and production records were current.

We completed walk around and reviewed flow of fluids with the well technician.

At the entry of the lease road I could not hear any noises coming from the pad. Upon entry of the pad the compressor can be heard operating.

Annular pressure monitoring was present the 13.375-inch diameter surface casing had 0 pounds per square inch and the 9.625-inch diameter intermediate had 0 pounds per square inch. The production equipment consisted of twelve 400-barrel tanks, three gas production units, one heater treater, vertical vent scrubber, and a combustor. The compressor that was the cause of the complaint has been removed.

1412102971 2014-09-29 Production Wells No Violations I walked through the production facility with Kevin Eshbaugh the Production Forman. There are three wells on pad that are producing. The reason for my visit was based on a complaint # 4818. There was no vibration coming from the pad.

Twelve roll-offs were tested and will be sent to the landfill. Replacement of stone and compaction was almost complete with two more loads of stone on the way. Hydro seeding of berm will take place on Friday. Tom Bauer will furnish ODNR Hull & Associates full analysis on sampling that was completed.

Had good returns of 10bbls and cement stayed to surface.
