0177760535 2014-10-28 Completion Testing No Violations On the day of my inspection accompanied by ODNR Regional Supervisor Rocky King and Inspector Justin Smith, I witnessed that the investigation was completed with plans to begin clean-up. Completed walk around with Company Man Will Ochampaugh and made recommendations for addressing secondary containment.
0380034575 2018-06-11 Production Wells No Violations The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Pressure monitoring for all casing strings were present. The production equipment consisted of two 400 barrel tanks, four sand traps and four gas production units. The identification was located at the wellhead.
0384967323 2015-01-27 Preliminary Restoration No Violations On the day of my inspection I witnessed that the production equipment included a sand trap, a GP unit and 2 400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. The site passed preliminary restoration according to ORC 1509.072A.

Eclipse was instructed after their investigation to move all flowback tanks, pull up secondary containment from the wells to southwest side of the pad remediate stone under flowback tanks and reinstall secondary containment prior to any future flowback.

0786904254 2016-09-29 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with a plunger lift system with valves in the open position. Gauges present read 645 pounds per square inches on the casing and 610 pounds per square inches on the tubing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater, a GP unit and 2-400 barrel purge tanks within containment and shared with the 8H, 4H and the 2H. Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF for sales.
0923484183 2015-06-19 Production Wells No Violations On the day of my inspection accompanied by Regional Emergency Operations Manager Trisha Cartmell, we reviewed the incident that occurred on October 25, 2014 where a flash fire occurred during flowback of well injuring one individual. Tubing pressure was at 1250psi and Casing pressure was at 1550psi. Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF which was inspected following inspection of well pad.

The well was equipped with a plunger lift system with gauges present reading 900psi on the casing, 750psi on the tubing and valves in the open position Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF.
