0092107913 2017-07-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a plunger lift system with valves in the open position. The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 275 pounds per square inch on the tubing and 385 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current. An Above Ground Storage Tank (AST) was present on the northwest corner of the well pad supplying fresh water to the Frank Miller pad for frac with support being provided by Great Lakes. Water was field tested revealing chlorides of <33 parts per million, pH 7 and 60 degrees.

Gauges present read 1285psi on the tubing and 1495psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. Discussion was held on the readings with Eclipse Lease Operators Bradley Paisley and Danny Hreha.

1305230628 2018-06-11 Production Wells No Violations The purpose of my inspection was a status check on a preliminary restoration. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 797 pounds per square inch, 1146 pounds per square inch on the production casing, 600 pounds per square inch on the 9.625 inch diameter intermediate casing, and 0 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of two 400 barrel tanks, six sand traps, six gas production units, and one indirect line heater. An above ground storage tank was situated in the Northwest corner of the pad. The identification was located at the wellhead.

Tubing and Casing pressure was at 2700psi with the intermediate casing at 80psi and surface casing at 0psi. The site passed preliminary restoration according to ORC 1509.072A.
