I talked to Randy Matheny he said they ran to bottom of hole and no cavings.Well allowed to flow into 300 Bbl frac tank, filled tank with apprx. 2 ft of fluid and shut back in. Randy plans to start work on tank battery.

Cleanup area that still is open and needs fill has surface water standing in it.

1102525090 2004-12-08 Production Wells No Violations Well has valves installed and no longer hooked up to frac tank. Gauge on tubing reads 340psi, annulus gauge reads 600psi. Some new grading next to well. Area is very muddy and surface water standing in places. Gas bubbles coming from fluid around wellhead next to outside of 4 1/2 casing. Frac tank still on location may be 1/3 full ? Dozer on site. Took 8 digital photos. Operator for safety and security reasons should have locks on well valves, or bull plugs, and also on frac tank.

The well was under control when I arrived and the crew was running production casing. Oil had filled the pit and run out to the north of the pits. The local EMA, Fire Dept, and the OEPA has been notified were on the site when I arived. Efforts to stop the flow into the stream north of the rig have been put into place.
1107189418 2004-09-20 Drill / Deepen / Reopen No Violations Inspection conducted with Tom Tugend, Deputy Chief. Complaints were received regarding strong gas smell in area of new drilling location. Spoke with some residents in the area. Spoke with drillers on site and they indicated that an oil/gas zone was drilled into at a shallower level than expected. Remedial actions had been taken by the drillers to contain oil that had flowed out of drilling pit by constructing a berm around the pit. Gas meter readings were also taken while on site.

3:20 PM the well cycled on the flair came on as it should, no vapors detected around the well head or tanks. Some vapors coming from the carbon filters. No noticeable odors. Gas detector reading 0 H2S, 0 LEL.

The operator has just completed painting the pump jack green and orange, storage and separator gray and all valves yellow. The well is pumping by a electric powered Baker pump jack and the well pad is clean and free of excess contamination. The storage consists of one production tank (#136735) and a steel brine tank set in a gravel and plastic lined fenced dike. The horizontal separator sets outside the storage between and well and tanks and the well identification and emergency contact information is clearly marked on tank #136735.
