The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
0562210346 2013-06-05 UrbanPreliminary Restoration No Violations The unit consists of a producing wellhead and tank battery, both of which are in chain-link enclosures and locked. The wellhead consists of a mechanical lift unit and is producing. The annular space is exposed and plumbed into production. The tank battery is comprised of 1 140 barrel tank and a 1 25 barrel poly tank. HB 278 equipment is in place and the secondary containment is good. The interior spacing is in accordance with the rules. There is also a Dominion metering run in the battery. I did not observe any problems or violations. The unit passes preliminary restoration.

1839005571 2017-10-20 Final Restoration No Violations I inspected this well location as an evaluation for Final Restoration. Plugging and abandonment was completed on 8/10/2015, witnessed by a ODNR Division of Oil and Gas Inspector, and a plugging and abandonment report was filed. At the time of this inspection, the former location of the well had been regraded, and was being utilized by the industrial facility for storage. The former production elements, including the flow-line, were removed and there was no remaining system elements. No further action is needed. Final Restoration is passed.
