0000988915 2016-05-11 Production Wells No Violations On the day of my inspection I found that the siphon dam was still in place at the stream head and the booms were still in place at the final collection pool. There was a film of residue on the surface at both points. The rest of the stream was running clear with no visible sheen or pockets of crude oil. The marshy area had been treated with Bio Sorb. I did not observe any sheen or crude oil in the water at the marshy area. The impacted soil was still staged inside of the dike. Coshocton Pipeline had brought in a load of new soil to reinforce the front wall of the dike. I did not observe any type of fluid leaching from the dike. I called Dwayne McElhany of Coshocton Pipeline to ask about the dike. He said the they were waiting for instruction from Jim Aslinides, owner of MFC Drilling, to move forward with the dike excavation. I suggested that Dwayne bring a water truck in to skim the last of the residue from the surface of the two pools in the stream, and then remove the siphon dam and all booms to allow the stream to flow naturally.
0296089098 2016-08-09 Production Wells No Violations Emergency Operations Regional Manager Trisha Cartmell requested that I follow up on the removal of the siphon dam and absorbent booms in the stream that was affected when the spill occurred at the tank battery. On the day of my inspection I found that the dam and booms were still in place. I called and left a message with Dwayne McElhaney of Coshocton Pipeline to request that they be removed.
0853271668 2016-05-23 Production Wells No Violations On the day of my inspection I found that the production tanks were still in place, and nothing had been done to remediate any of the contaminated soil in the dike. I spoke with Josh Schiering who claimed that the attempt to remediate in place was unsuccessful and MFC was now going to excavate and haul the contaminated soil away. There was a small amount of fluid leaching from the front of the dike, but nothing had run down the hill or made it anywhere near the culverts. The siphon dam and absorbent booms were still in place in the stream. I found no residual oil contamination in the stream. Pumper Ray Dillon showed up while I was at the site. He confirmed the plans to haul the soil to a landfill, but he did not have a solid time frame.
0868439587 2016-05-02 Production Wells No Violations On the day of my inspection I found that the impacted soil that had been excavated outside of the dike was still staged for removal inside of the dike. I found no signs that any oil was leaching through the dike walls. The grass and weeds in the marshy area at the stream head still needed some clean-up. The water was clear of any oil pockets or sheen. The siphon dam was still in place. There was a small amount of residue that had collected against the boom at the dam. All other booms and pads had been removed from the stream from the dam to the collection point approximately 1200 down stream. I found no oil pockets or visible sheen throughout the course of the stream. There was some sheen present and residue that had collected at the pool where the remaining boom was set up. I spoke with Nick Hahn of Coshocton Pipeline to suggest that they treat the remaining oil in the vegetation around the marshy area with an oil absorbent material such as BioSorb.

I called Nate Hahn of Coshocton Pipeline, who said that he didnt know when they would be out to work on it. He also claimed that they have been authorized to flush the gravel and soil in the dike rather than excavate and haul away. The siphon dam was still in place at the head of the stream. I found no signs of residual oil throughout the course of the stream.

1452702972 2016-01-13 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped with a pumping unit. The electric motor was intact, the valves were open and the unit was in operation. The tank battery consisted of one 210 barrel tank, one 140 barrel tank, one 100 barrel tank and an earthen dike. There were two separators outside of the dike. Identification was posted and I found no leaks or spills. The tank battery is shared with permit 7172.
1461853264 2016-04-26 Production Wells No Violations I was contacted at 11:00 by Jim Aslanides of MFC Drilling to report a spill outside of containment. The initial report was that a brass load valve had failed on a 210 barrel tank, releasing approximately 60 barrels of brine and 8 barrels of oil into the earthen dike. It was also reported that approximately 2 barrels of oil had ran over the dike wall and made it into a stream. I called the Emergency Operations duty number to notify Scott King. He then notified Joshua Schiering who met me at the site. When I arrived on location I spoke with pumper Ray Dillon and contractor Dwayne McElhany. Ray said that the spill was discovered on 4-25-16 around 16:00. I found that the fluid in the dike had been vacuumed out with a water truck. The soil and gravel inside of the dike was contaminated with brine and crude oil. There was a visible path from the dike, downhill to a drainage ditch running parallel with the lease road that was stained with oil, and there were small pools of oil standing in the ditch. The fluid then left the ditch and entered a culvert collection point that was thick with cattails and woody vegetation. The flow entered the culvert, under TR 256 and into a small intermittent stream running east to west toward Center Creek. The crew from Coshocton Pipeline, a contractor for MFC, had set up absorbent booms and pads at various points along the stream. Upon walking the stream I found that the pockets of crude oil were mostly contained by the booms, and the visible sheen dissipated approximately 1500 feet downstream from the point of origin. I did not observe any impacted fish or wildlife. Josh and I collected water samples from the entry point and at the furthest point where there was a visible sheen to field test for chlorides. The chloride content in the sample at the entry point was under 200 ppm, and the chloride content in the sample 1500 feet downstream was negligible. MFC was informed that they must notify Ohio EPA of the incident. I spoke with Trevor Irwi
1464788319 2016-05-31 Production Wells No Violations On the day of my inspection I found that Dwayne McElhaney and Nick Hahn of Coshocton Pipeline were on location removing the tanks from the dike in order to haul the contaminated soil to a landfill. There was a 10 to 12 layer of gravel in the bottom of the dike under the tanks.

1694051261 2016-06-06 Production Wells No Violations On the day of my inspection I found that the 210 barrel and 100 barrel steel tanks had been reset and the earthen dike had been reconstructed. There was a new layer of gravel in the base and the walls had been built up 3 to 4 feet. I found no remaining crude oil contamination inside of the tank battery. Most of the vegetation was starting to regrow in the marshy area where the culverts come together. The siphon dam and absorbent booms were still in place at the head of the stream and at the final collection point from the clean-up. I spoke with Josh Shiering with Emergency Response and he said that he gave MFC the clearance to remove the dam and the booms. He also said that they were allowed to keep the impacted soil in a containment area on top of the hill where they were bio-remediating on site with lime and fertilizer.

2054085137 2016-04-27 Production Wells No Violations On the second day of the clean-up I found that the amount of visible crude oil in the stream had been significantly reduced. Coshocton Pipeline had installed a siphon dam at the point of entry, and there were fresh absorbent pads and boom at various intervals along the stream. The crew had concentrated a high volume of booms and pads at a collection point that was reinforced by a natural dam of woody debris. I found no visible crude oil or sheen downstream from that final collection point. The crew was instructed to remove the booms between the siphon dam and the final collection point in order to allow the stream to flow at a higher rate. They also walked the stream with shovels and a leaf blower to push the remaining oil to the collection point where it could be pulled up with a vacuum truck. Dwayne McElhany removed the layer of top soil that had been impacted outside of the dike and staged the soil in the dike for removal. He also removed the impacted soil in the drainage ditch. Once the stream has remediated, Coshocton Pipeline will focus their efforts on removing the storage tanks and excavating all contaminated soil from inside the dike to be hauled to an approved landfill.
