Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0398998567 2018-06-04 Production Wells No Violations On the day of my inspection I found that there were no changes in the condition of the well or tank, as far as configuration. They were starting to become overgrown with vegetation. There was a plugging permit issued for this well on 2/23/18. I called Jim Alsinides to ask what his plans were and he did not answer, so I left a voice message. He returned my call and said that they were still working with Battelle on testing the well for carbon sequestration. Jim said that he has been working with Battelle geologist Amy Lang and he has sent documentation to the state.
1297588345 2018-09-18 Production Wells No Violations On the day of my inspection I found that there were no changes at the well site. There was a plugging permit issued for this well on 2/23/18. I called Jim Alsinides to ask what his plans were and he did not answer, so I left a voice message. He returned my call and said that he has not heard anything recently from Battelle on testing the well for carbon sequestration. Jim claimed that the well had gas pressure on it, but he could never get any oil out of it. He also made the remark that the gas was high in nitrogen. Jim said that he will try to get an update from Battelle and get back to me. He has not committed to plugging the well at this time, and the he does not have any plans to produce the well at this time.
1507831833 2017-10-12 Production Wells Violations Noted On the day of my inspection I found that the well was equipped with a 4.5 to 2 inch diameter swage, to 2 inch steel line, through a regulator and plumbed directly into a 100 barrel steel tank. The tank was less that 10 feet from the wellhead. The valve on the swage was open and topped with a gauge that showed zero (0) psi gas pressure. There was an identification sign on the tank for MFC that was blank. There were no leaks or spills around the wellhead or tank. The annual production reports are up to date, but the only production ever reported since 1995 was 100 thousand cubic feet of gas in 2010. This well is not being commercially produced, and there were no connections for domestic gas. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1557937032 2019-05-15 Production Wells No Violations On the day of my inspection I found that there were no changes with the configuration of the well. The production casing was still equipped with a 4.5 to 2 inch diameter swage, and directly plumbed to the 100 barrel steel tank. The tank was still set within 10 feet of the well. The valve on the swage was open, and there was a gauge on top of the swage that showed zero (0) pounds. I was not positive that the gauge was working though. I called Jim Aslinides to ask what his plan was. He claimed that the well is capable of viable production and it has close to 1000 pounds of gas pressure. I asked Jim to come up with a way to prove his claim and to come up with a plan for future use. I also suggested that he look into the temporary inactive status until they get the infrastructure in place to produce the well.

1721476883 2018-02-23 Production Wells No Violations On the day of my inspection I found that there had been no changes at the well site. The well was still equipped with swage and plumbed directly to a 100 barrel tank. The valve was open on the swage and the gauge showed zero (0) pounds. The condensation line on the tank indicated that there was approximately 60 barrels of fluid inside. The identification sign was still blank. There were no signs that any work had been done to make the well capable of production. I have not received a plan of action for the well and I have not received a permit to plug and abandon. I spoke with Jim Aslinides, who claimed that he has not acted to resolve the compliance notice because Battelle had expressed interest in the well for carbon sequestration research. I reminded Jim that I had requested a written plan of action to be submitted within 30 days of the receipt of the compliance notice. I asked that he get his plan mailed or emailed to me as soon as possible.
