0133696131 2012-10-17 Production Wells No Violations Follow up to NOV 1702051950. The tank across the lease road from the well had belonged to Broadstreet Energy all along, as per: Murphy Oil. Apparently Murphy Oil had contacted Broadstreet and notified them of the leak in the bottom of that particular tank. I was under the impression the tank belonged to Murphy Oil because it was in the dike with two meters that were labeled with Murphy Oil ID tags. It was a shared dike between the two companies. The oily soils caused by the leaky tank had been removed and new soil was put in its place, the leaky tank that belonged to Broadstreet Energy had been removed. I also spoke with the Field Manager for Broadstreet Energy over the phone and he verified all of this to be true.

There is a small puddle of crude oil and fluids around the base of the tank near a 2 nipple. The puddle is contained inside the dike.

1702051950 2011-04-11 Production Wells Violations Noted Common tank battery with Pinkerton #450 2-3648. Upon inspection tank battery was found to be improperly indentified and a small leak was noticed on the backside of the production tank at the bottom. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tank each well-head shall be identified as to owner, permit number where available, well number and lease name. Leaking tank shall be repaired or replaced.
