2013-07-11 H2O 0 15563 Gal 0 Universal Well Services Inc. Universal Well Services conducted a cross linked hydraulic fracture stimulation of the storage well. The well was recently reworked. Nitrogen was used to decrease water requirements and flowback. Nitrogen comprised 20% of the fracture fluid by volume. The liquid portion of the fluid was cross linked gel using guar. This job was partially witnessed due to receiving water samples from Tom Benko for the DNR lab. No acid was used on this job.

Cement has fallen away from surface in the annulus between the production casing and surface casing. I contacted Terry VanSickle of Columbia Gas who advised that he will get additional cement to fill up the annulus. Pit needs to be pumped out.

The well is equipped with a flanged wellhead with 1-master valve and 1-wing valve, a drip, and 1-210 gal plastic tank. Well identification is clearly displayed. The well is located ~115 from its RBDMS location. A map showing the location discrepancy, as well as a request for the location to be updated in RBDMS was sent to Aaron Lantz (DOGRM Columbus). No other problems were noted at the time of the inspection.
