2830864 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted I arrived on site to conduct a routine complete investigation in response to a site restoration extension request received from the Operator. The purpose of this inspection was to ensure the two sediment traps onsite had been converted into infiltration berms and an additional infiltration berm installed along the access road.

After performing an administrative review of the plan and a site inspection, I agree that the aforementioned conditions have been fulfilled.

During the inspection, I noticed several of the tops on the annular vent tanks had cracked. Operator, please inspect the area and repair any deficiencies observed.
