2258621 Primary Facility 2014-03-21 Drilling/Alteration Pending While in the process of walking around the perimeter of the drilling rig and associated secondary containment system, Mr. Hoehle and I observed small pockets of water off containment on the slope behind the backyard and oil based mud tanks in the southwest section of the Well Pad. We noticed the water was seeping to the surface and it appears as if the underground seeps originated under the secondary containment system. The ground was soft and staining was observed. Water was actively flowing to the surface of the ground on the slope near the oil based mud tanks. Conductivity readings were (9) millisiemens/cm. We observed more standing water off containment as we continued to walk around the perimeter of the secondary containment system. I recorded conductivity readings of (5) milliSiemens/cm in water on the surface of the ground in the southwest corner of the Well Pad near the Strad Energy oil based mud tanks.
2273817 Primary Facility 2014-05-23 Administrative/File Review Violation(s) Noted The Department is issuing a Notice of Violation based on the sample results of water seeping to the ground surface of the Well Pad, a soil sample collected next to the seep and standing water on the ground surface of the Pad near the oil based mud tanks in the southwest corner of the Pad. The samples were collected on 3/21/2014 during an inspection of the site; inspection record number (2258621). The sample locations and descriptions are listed below in the inspection report. PADEP Bureau of Laboratories in Harrisburg, PA issued the sample results to the Department for sample number (164) on 4/18/2014; sample number (165) on 4/4/2014; and sample number (166) on 5/21/2014. Sample numbers (164) and (166) are water samples using the PADEP Bureau of Laboratories Standard Analysis Code (946). Sample number (165) is a soil sample using the PADEP Bureau of Laboratories Standard Analysis Code (961).

This inspection was performed in response to a well site restoration period extension request received from the Operator on 11OCT2018. The purpose of the inspection was to ensure the proper installation of PCSM BMPs on site, namely two infiltration berms.

After reviewing the site plan and performing an inspection, I determined that both infiltration berms are implemented correctly with regard to the well site restoration extension request submission received by the Department on the above-mentioned date.
