2619142 Primary Facility 2017-07-25 Routine/Complete Inspection No Violations Noted At the time of inspection drill rig and drilling equipment has been removed from site. Secondary containment in place and appears to be structurally sound. Activity related to pipeline work ongoing. No E&S issues.

Previously issued violations against the ESCGP for the site remain outstanding (ID 2555763).

2623313 Primary Facility 2017-08-08 Routine/Complete Inspection No Violations Noted Inspection performed in response to electronic notification of the resumption of drilling. At the time of inspection, drill rig on site and drilling related activity appears to have commenced. All drilling equipment and materials located and stored within secondary containment. During inspection Department observed, what appears to be remains of a spill on to the secondary containment on which the drilling mud returns containment tanks are located. Site foreman confirmed a small release of drilling mud within the secondary containment. The spill was immediately cleaned up in accordance with policy. Additionally, the spill was completely contained by secondary containment and thus is in compliance with the reporting requirements of 78a.66. Drill permits, ERP and PPC plans located on site and appears to current.

2626379 Primary Facility 2017-08-14 Drilling/Alteration Violation(s) Noted Rig was making a bit trip from 9,981' at the time of my inspection. Operator encountered a geologic upthrust fault and drilled from the targeted Marcellus formation into the Onondaga formation without a conservation well drilling permit and an NOV is being issued. Daily drilling information was up to date and all equipment on site looked ok.

2644059 Primary Facility 2017-10-05 Routine/Complete Inspection No Violations Noted At the time of inspection drill rig not on site and drilling activity appears to have ceased. All equipment and materials located within secondary containment. No E&S issues.

2717458 Primary Facility 2018-04-12 Routine/Complete Inspection In Compliance with Policy Inspection in response to receipt of notification of the resumption of drilling. At the time of inspection drill rig being set up. Secondary containment on well pad in place. All materials and production equipment located within secondary containment.

At the time of inspection Department observed what appears to be a diesel fuel spill to gravel pad adjacent to diesel fuel tank. Visible staining and pink residue of the gravel observed. Characteristic petroleum odor to stained gravel also noted. Department notified site foreman and clean up was initiated immediately. Department requests Operator notify Department as to the cause of spill and volume released.

Additionally fuel tank and secondary containment improperly staged, not level with the ground. Department recommends Operator address the staging of the fuel tank and secondary containment ensuring that it is placed level with the ground.
