2492938 Primary Facility 2016-06-13 Routine/Complete Inspection In Compliance with Policy At the time of inspection well appears equipped for production. Well tag present and affixed in a permanent manner. Brine tank located on site. During the inspection Department observed a slow drip at the base of the brine tank. No immediate impacts of dead and/or distressed vegetation observed as a result of this. Department spoke with Mark (Operator) and informed of above findings. Recommended Operator follow-up with Department upon investigation of reported concerns and to quantify release and identify all materials released if applicable within 24 hours.

Department will conduct a follow-up inspection. No E&S issues in relation to site and/or access road.

2495023 Primary Facility 2016-06-22 Administrative/File Review No Violations Noted Operator (Mark) contacted Department on 6/22/16 @ 8:30am to follow-up on inspection ID 2492938. Operator confirmed Department findings and estimates release to be less than a gallon. Visibly impacted material directly beneath leak has been excavated and cleaned-up and disposed in accordance with policy. Brine tank leak has been fixed and put back into operation.
2498949 Primary Facility 2016-07-01 Routine/Partial Inspection No Violations Noted Inspection performed as a follow-up to inspection ID 2492938. At the time of inspection the slow drip from the base of the brine tank has been resolved. Site appears stable with no E&S issues in relation to site and/or access road.
