2684246 Primary Facility 2018-01-12 Incident- Response to Accident or Event No Violations Noted On 1/12/18 an incident inspection was conducted at the Lesnick Well Site in response to an operator reported BMP failure accompanied by Range Representative Hannah McAvoy. The following was observed:

>>The site was under construction at the time of inspection with perimeter controls and surface stabilization along the completed section of the site access road showing wear and some damage from recent heavy rain events.

>>Sediment laden water was observed to be leaving the site from around a drain pipe. Modification of surrounding controls to prevent surface flow from bypassing is recommended. This constitutes a violation of 25 Pa. Code 78.53, 25 Pa. Code 102.4(b)(1), and 25 Pa. Code 102.11(a)(1) in that the operator failed to successfully design, implement, and maintain site BMPs.

>>The primary reported BMP failure occurred at the junction of the diversion channel and sediment pond. The breach occurred at a weak or low spot between the diversion channel and the sediment pond. At the time of inspe

2768100 Primary Facility 2018-08-27 Routine/Complete Inspection In Compliance with Policy On 8/27/2018 at approximately 1000, I conducted an inspection of the Lesnick John Well Site located in Mount Pleasant Township, Washington County in response to a reported spill of antifreeze to the graveled well pad. I met with Hannah McAvoy on site to discuss the spill and associated remediation.

On 8/24/2018 at approximately 2245, a water truck experienced a hose clamp failure on its radiator which resulted in a release of approximately 9 gallons of antifreeze to the graveled well pad. Absorbents were placed on the affected area and Specialized Professional Services Inc. (SPSI) responded and remediated the impacted area placing it into one roll-off box. The affected area appeared to be remediated according to plan during the inspection.

While inspecting the well site, the following was observed:

  • The well pad was being prepared for well stimulation activities which should begin to occur at the end of the calendar month.
  • The Act 9 sign located at the site entrance was in good condition.

2815314 Primary Facility 2018-12-14 Compliance Evaluation No Violations Noted On 12/14/18 a compliance evaluation inspection of the Lesnick John Well Site was conducted in response to an incident and violations documented on 1/12/18. The following was observed:

>>The site of the violations was fully remediated with permanent stabilization and appropriate BMPs in place. No evidence of erosion or residual sediments was found.

>>The site was in overall good condition with no violations noted.
