Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2612382 Primary Facility 2017-07-05 Preoperation Inspection No Violations Noted Today's pre- operation inspection was conducted at the Munce well pad under the requirements of Act 13 section 3258(a.1).

The perimeter erosion and sedimentation controls were installed in accordance with ESG15-125-0073 prior to my inspection. The site was stable and the perimeter erosion and sedimentation controls were adequate enough to contain the erosion and sediment within the active work site. Operator can now begin to move in equipment and commence drilling

2615855 Primary Facility 2017-07-17 Drilling/Alteration No Violations Noted Lightning Energy auger rig

Drilled 30” wellbore and set 26” conductor casing at 40 ft. on 7/13/17.

2658293 Primary Facility 2017-11-07 Drilling/Alteration No Violations Noted Patterson Drilling Rig #343 moved onto this well on 10/30/17 and resumed drilling at 6652 ft. KB. KOP was at 6181 ft. (with air rig). EOB was at 8620 ft. MD (6298 ft. TVD). TD was at 17,984 ft. MD (6965 ft. TVD) on 11/6/17. Currently running 5 1/2" casing.
2663773 Primary Facility 2017-11-22 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #343. TD was at 17,984 ft. MD (6965 ft. TVD) on 11/6/17. The 5 1/2" casing was set at 17,962 ft. The estimated TOC is at 5181 ft.
2720971 Primary Facility 2018-04-18 Follow-up Inspection No Violations Noted On 4/18/18 a follow up inspection of the Munce 10060 Well Site was conducted in response to questions related to a incident conducted of the same site on 4/17/18. Five GPUs were verified as present on site at the time of inspection.
2721310 Primary Facility 2018-04-17 Incident- Response to Accident or Event No Violations Noted On 4/18/18 an incident inspection of the Munce 10060 Well Site was conducted in response to an operator reported spill of 27 gallons of diethylene glycol (DEG). The following was observed:

>>Range Resource's Derek Zofchak reported that on 4/16/18 a routine inspection found DEG dripping from a loose fitting on the #9 GPU. The total leak volume was estimated at 27 gallons based on comparison of initial fluid fill level to remaining fluid. Weavertown Environmental Group was reported as on site working to complete cleanup roughly one hour after the spill was discovered. This constitutes a violation of 25 Pa. Code § 78.54 and 25 Pa. Code § 78.57(a) in that the operator failed to control regulated materials in a manner that prevents prevents pollution to the ground or Waters of the Commonwealth.

>>Site inspection found that all visually impacted materials from the area around the leak had been removed, the excavated area was consistent with the reported spill volume.

>>GPUs were found to have insufficient

2749416 Primary Facility 2018-06-19 Compliance Evaluation No Violations Noted On 6/14/2018, a follow up inspection was conducted in response to an operator reported spill (CTS ID: 334975), the following was observed:

>>Three roll off boxes remained on site at the time of inspection in the immediate vicinity of the area of the spill. The excavation for the spill cleanup remained open and was partially filled with water. Field testing of the water indicated normal ground water conductivity between 300-500µS. Cleanup appears to be in compliance with policy. No violation.

2753830 Primary Facility 2018-07-18 Routine/Partial Inspection In Compliance with Policy On 7/16/2018 at 1712, a spill was reported to the PA DEP hotline by Range Resources which had occurred at 1600 on 7/16/2018. On 7/18/2018 at approximately 0930, I conducted an incident response inspection. I met with Nicholas Reda (Range Resources) to discuss the incident and associated remediation.

Approximately 10 gallons of produced water were released to the graveled well pad under previously set containment. The cause of the release remains unknown. Specialized Professional Services Inc. (SPSI) responded and remediated the impacted area placing it into one roll-off box.

The wells on site were clearly marked and within secondary containment. The production fluid/condensate tanks were within secondary containment and appeared to be in good condition. The GPUs on site appeared to be in good condition and had secondary containment.

The spill was addressed in compliance with the spill policy.

2825851 Primary Facility 2019-01-11 Compliance Evaluation No Violations Noted On 1/11/19 a compliance evaluation inspection of the Munce Site was conducted in response to violations noted on 6/14/18. The following was observed:

>>No residual impact was found in the area of the spill.

>>The site was found to be in good condition with no violations found.

>>Minimal activity was viewed on site at the time of inspection.
