2587981 Primary Facility 2017-04-27 Preoperation Inspection No Violations Noted On Thursday, April 27, 2017, I conducted a pre-operation inspection at the Thai Volcano Well Site located in North Bethlehem Township, Washington County. The inspection was conducted in response to an operator request in accordance with Act 13, Section 3258 (a.1)The Oil and Gas Act. Mr. James Davis, representing Rice Energy, was also present during the inspection.

The Act 9 sign was in place and clearly legible at the site entrance and all required documentation was available on-site. The access road, which was recently treated for dust suppression, and the well pad are in good condition.

Various stages of vegetation were observed throughout the site which was stabilized with a combination of seed, mulch, erosion control blankets and vegetative growth. All perimeter BMPs have been installed and are functioning as designed. The wet pond on the southern end of the site has been competed and remains to be seeded. The seed for the wet ponds has been ordered and is expected to arrive within the week. The
