2582639 Primary Facility 2017-04-13 Preoperation Inspection No Violations Noted On Wednesday, April 12, 2017, I conducted a pre-operational inspection at the Bovinator Well Site located in Amwell Township, Washington County. The inspection was performed following an operator request in accordance with 58 Pa. C.S. Section 3258 (a.1). Mr. James Davis, representing Rice Energy (Rice), was also present during the inspection.

The Act 9 sign was in place and clearly legible at the site entrance and all required documentation was present on-site. The access road and well pad were in good condition. The stream crossing was in good condition and constructed according to the plan. At the time of the inspection, Rice was in the process of installing concrete safety barriers along the access road in the vicinity of the stream crossing for driver safety during inclement weather which will further protect the stream as well.

All perimeter erosion and sedimentation controls have been installed according to plan and are functioning as intended. The operator should continue their periodic monit

2601695 Primary Facility 2017-06-07 Drilling/Alteration No Violations Noted Butch’s Rathole Service auger rig

Drilled a 36” wellbore and set 26” conductor casing at 50 ft. on 6/6/17.

2728621 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Bovinator well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The site is not producing at this time. The wells were spudded and covered with a steel plate. No production equipment onsite.

2753768 Primary Facility 2018-07-19 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349 moved onto this well on 7/14/18 and resumed drilling at 87 ft. KB. The 18 5/8" casing was set at 331 ft. on 7/16/18 and cemented to surface with 28 bbls returned. Currently at 575 ft. and setting cement flash plugs prior to running 13 3/8" casing. A mine was found from 542 ft. to 549 ft. Three flash plugs have been dropped. The first was 80 bbls. The second and third plugs were each 60 bbls. Mine is still flaring. Operator will drop another plug.
2756324 Primary Facility 2018-07-26 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349. The 13 3/8" casing was set at 597 ft. on 7/24/18. The operator displaced cement to surface with 29 bbls returned. The cement dropped to 130 ft. The casing was then grouted to surface. A mine was found from 542 ft. to 549 ft. Two baskets were set at 445 ft. A packer was set in the 13 3/8" string from 453 ft. to 467 ft. The operator stopped drilling at 649 ft. and moved to the next well.
2759593 Primary Facility 2018-08-02 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349 reentered this well on 7/31/18 and resumed drilling at 649 ft. The 9 5/8" casing was set at 2037 ft. Currently at 2080 ft. and rigging up to cement casing.
2762060 Primary Facility 2018-08-09 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349. The 9 5/8" casing was set at 2037 ft. on 8/2/18 and cemented to surface with 40 bbls returned. KOP was at 4992 ft. EOB was at 8089 ft. MD (7445 ft. TVD). TD was at 12,919 ft. MD (7481 ft. TVD) on 8/8/18. TOH after conditioning well bore. Currently preparing to run 5 1/2" casing.
2765717 Primary Facility 2018-08-21 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349. The 5 1/2" casing was set at 12,909 ft. on 8/9/18. Estimated TOC at 3425 ft.
