2582613 Primary Facility 2017-04-13 Preoperation Inspection No Violations Noted On Wednesday, April 12, 2017, I conducted a pre-operational inspection at the Bovinator Well Site located in Amwell Township, Washington County. The inspection was performed following an operator request in accordance with 58 Pa. C.S. Section 3258 (a.1). Mr. James Davis, representing Rice Energy (Rice), was also present during the inspection.

The Act 9 sign was in place and clearly legible at the site entrance and all required documentation was present on-site. The access road and well pad were in good condition. The stream crossing was in good condition and constructed according to the plan. At the time of the inspection, Rice was in the process of installing concrete safety barriers along the access road in the vicinity of the stream crossing for driver safety during inclement weather which will further protect the stream as well.

All perimeter erosion and sedimentation controls have been installed according to plan and are functioning as intended. The operator should continue their periodic monit

2728610 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Bovinator well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The site is not producing at this time. The wells were spudded and covered with a steel plate. No production equipment onsite.
