2634595 Primary Facility 2017-08-28 Incident- Response to Accident or Event Violation(s) Noted On August 28, 2017, the Department performed an inspection of the Southwestern Energy Marlin Funka well site. Present at time of inspection was Josh Boyda (SWN). The site is located in West Finley Twp., Washington County. The inspection was performed as a response to a reported brine release by the operator.

The operator reported approximately 18 barrels of brine was released to secondary containment around the produced water tanks on site with approximately 1 barrel discharging to the well pad. The Department was notified of the release on 8/27/17 by phone at 12:45 p.m. At approximately 10:00 a.m. a valve in the 9H GPU failed causing a release of high pressure gas and fluid to the production fluid tanks on site. SWN presonnel on site activated the ESD which shut in the well. Approximately 18 barrels of brine were released out of the pressure release valve on the storage tank into secondary containment with 1 barrel being released to the ground. The site is located in West Finley Twp., Washington Co. The inspection is a compliance evaluation associated with a brine release and violations noted on 8/28/17.

The operator has excavated impacted material near the brine storage tanks on the well pad. The operator has back filled the area with new stone after taking confirmatory samples to ensure remediation of the brine release. The impacted material excavated from the well pad is being stored in a roll off box in secondary containment for disposal.

The site is stable with greater than 70% vegetative cover except for a temporary access road constructed by Williams on the east side of the well pad. The well pad and access road are in good condition. The operator has placed the GPU on site in secondary containment.

Present at time of inspection was Kevin Maskol (DEP). The site is located in West Finley Twp., Washington Co. The inspection is a routine inspection of the site.

The operator has excavated impacted material near the brine storage tanks on the well pad. The operator has back filled the area with new stone after taking confirmatory samples to ensure remediation of the brine release.

The site is stable with greater than 70% vegetative cover except for a temporary access road constructed by Williams on the east side of the well pad. The well pad and access road are in good condition. The operator has placed the GPU on site in secondary containment.

2677080 Primary Facility 2017-12-21 Incident- Response to Accident or Event No Violations Noted On December21, 2017, the Department performed an inspection of the SWN Funka well pad site. The site is located in West Finley Twp., Washington, Co. The inspection was related to a reported brine release by the operator on 12/20.

The operator reported two spills for the 9H Utica well.

Spill 1 (Produced Water) 12/20 at approximately 10:45am, a Well Tender was blowing down the dehy tower on the Marlin Funka production pad, when he noticed fluid (produced water) being released from the top of one of the production tanks on the pad. It is believed that while belly dumping the dehy tower to bleed down the fluid, the produced water tank over pressured and sprayed about 2 bbls of water out the top of the tank. The well tender quickly shut the valve being used to bleed pressure off the dehy tower to stop the flow to the tank. 2 bbls of produced water was contained inside the production tank containment and 2-3 gallons sprayed out onto the ground directly next to the tank containment.
