2514593 Primary Facility 2016-04-08 Routine/Partial Inspection No Violations Noted On 4/8/16, I performed a routine partial inspection at the Master Splinter site in response to an operator request for review of minor modifications and made the following observations:

•The operator requested modifications adjusting the placement of a few pieces of CFS, a small adjustment straightening the access road to reduce disturbance, and removing two sections of CFS from areas that were no longer being disturbed. Listed modifications were approved and redlined into plans on site at the time of inspection. •General site construction had not begun at the time of inspection. •Necessary paperwork was present on site at the time of inspection.

2514719 Primary Facility 2016-05-18 Routine/Complete Inspection No Violations Noted On 5/18/16, I performed a routine complete inspection at the Master Splinter site and made the following observations:

•Required paperwork was in box. •No work was being conducted on the site and no operator personnel were present at the time of inspection. •Site perimeter controls were in place and appeared to be fully functioning at the time of inspection. •Disturbed or created slopes observed on site were generally stabilized, most flat areas of the site were still exposed. Work on site appeared to be ongoing as evidenced by large amount construction equipment present. No violation.

2514838 Primary Facility 2016-06-28 Preoperation Inspection No Violations Noted On 6/28/16, I performed a pre-operation inspection for the Master Splinter site in response to a request from the operator and made the following observations:

•All perimeter controls were in place and appeared functional. •Site vegetation was 50% or greater in most areas with no substantial erosion damage. One very minor erosion channel was found along the access road and remediation was initiated immediately. •All necessary paperwork was present on site at the time of inspection. •The operator conveyed a landowner request to remove a section of CFS in a vegetated area below the access road so that it could be used for a hay field. As the area was largely over 70% and the request was partially accepted with a recommendation to leave a portion of the CFS near the sediment trap.

Conclusion: •The site appears to meet all requirements for drilling to begin.
