On 2/5/2014 a call came in at 10:10pm that Allied was having trouble with the cement job. Pumped 200bbls or fresh water and 35bbls of gel, then 10bbls of fresh water. Next they started pumpling cement. At 95bbls of cement pumped, the pressure spiked to 2500psi while pumping at 4.5 bbls/min. Decided to bleed off pressure and try again. Pumpled 1.4 to 1.5 bbls/min. and pressure began to build again. CNX terminated cement job. 02/06/2014 CNX decided to grout casing to surface.using a 1" pour string. On 2/7/2014, 84bbls of cement was pumped throught pour string and got 1bbl to returns.

2584798 Primary Facility 2017-04-11 Routine/Complete Inspection No Violations Noted On 4/11/2017, the Department performed an inspection of the Noble Energy, NV60 site. The site is located in East Finley, Twp., Washington Co. The inspection was a routine inspection of the site post restoration and before termination of the ESCGP2 permit.

The site was recently transferred from CNX to Noble Energy. CNX had applied for a restoration extension for the site which has expired. CNX had restored the site and installed PCSM structures (Infiltration Berms). The PCSM structures are not functioning and are failing causing accelerated erosion. Several areas, recently restored on the well pad and well pad slopes have poor vegetative cover. Kim Walker (Noble Energy) was contacted and she stated that Noble is discussing reconstructing the PCSM structured or possibly modifying the approved ESCGP2 permit for the installation of different PCSM structures. Noble plans to revisit the site in the near future to address the deficiencies.

2631128 Primary Facility 2017-07-26 Routine/Complete Inspection No Violations Noted On July 26, 2017, the Department performed an inspection of the HG Energy NV60 well pad site. Present at time of inspection was Jared Stemple (HG). The site is located in East Finley Twp., Washington County. The inspection was related to a request by the operator to meet on site to discuss start rehabilitation of the PCSM structures and interim restoration of the site.

The operator has contracted with Burns Construction to rehabilitate the failing infiltration berms on the northeast corner of the site. ARM group will be overseeing the construction activity in the field for the operator.

9 wells have been drilled to completion and are equipped for production.

2643112 Primary Facility 2017-09-07 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the NV60HS well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained, no leaks or fluids around well head. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

2651223 Primary Facility 2017-10-04 Site Restoration No Violations Noted On October 4, 2017, the Department performed an inspection of the Noble/HG Energy NV60 well pad. The site is located in East Finley Twp., Washington Co. The inspection was related a well restoration report for the wells drilled on site by CNX prior to the sale of the site to Noble and then HG.

The well pad access road and well pad are in good condition.

The site has been restored and PCSMs (Infiltration berms) have been installed. The two infiltration berms on the northeast corner of the well pad are not functioning and have failed. The well pad has been reduced in size and vegetated. HG Energy has met with the Department and discussed repair of the infiltration berms.

9 wells have been drilled to completion and are equipped for production on site. The GPUs on site are not in secondary containment. The operator will need to place the GPUs into containment in the future.
