2513608 Primary Facility 2016-02-05 Complaint Inspection No Violations Noted On 2/5/16, I performed an incident inspection at the Kevech pad in response to a complaint concerning the site and made the following observations:

•Per the complainant:

    -The pad was built ten feet higher than permitted in order to not breach a coal seam which resulted in the cut slopes of the pad being steeper than permitted.
    -Multiple other pad features were suspected to be improperly constructed and maintained.
    -Concerns were also voiced about the grading and maintenance of the site access road.

•Pad elevation was measured to be 1080-1094 feet above sea level via multiple GPS devices, permitted pad height is 1085. •Site vegetation was noted to be sparse in areas, particularly north and east of the pad as well as on the southern and western cut slopes. •Badly damaged and partially destroyed remnants of super silt fence were found north and east of the pad. 2523975 Primary Facility 2016-04-25 Follow-up Inspection No Violations Noted On 4/25/16, I performed a follow up inspection of the Kevech site accompanied by DEP engineer Emmanuel Nwaobilo in response to complaint 317639 from 2/3/16 (initial inspection took place on 2/5/16) and made the following observations:

•Per DEP engineer Emmanuel Nwaobilo the site’s contours and elevation appeared to be built to permitted plan specification. No violation. •Several heavily vegetated soil piles noted around the base of the pad not found in site plans were added as minor modifications and redlined into plans. No violation. •A corrugated drain pipe was found leading from just below the south east corner of the pad down to the vegetated channel at the bottom of the slope. Erosion channels were found in two separate locations along this pipe. No violation, remediation recommended. •A moderate erosion channel was found entering a ponded area of the diversion channel. •The operator has noted that a number of minor modifications had previously been made and accepted by DEP personnel but documentati

2609262 Primary Facility 2017-06-08 Complaint Inspection No Violations Noted On 6/8/17 an RTNC inspection of the Kevech well site and impoundment in response to a report of waste water being dumped in the freshwater impoundment. The following was observed:

>>No evidence was found at the impoundment of improper dumping.

>>Live fish were seen swimming in the impoundment indicating water remains uncontaminated.

>>The Kevech pad was being used as a staging point for the nearby Harold Haywood pad at the time of inspection.

2728646 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted This well has been completed and is producing. Well site appears to be well maintained. Along with 7 producing wells 6 new wells are being spud.
2742924 Primary Facility 2018-05-09 Routine/Complete Inspection No Violations Noted On 5/9/18 a routine partial inspection of the Kevech pad was conducted in response to drilling notifications and reports of significant activity at the site. The following was observed:

>>Construction of an AST pad was being conducted at the time of inspection. Multiple sections of CFS located around the construction area were noted to be poorly installed leaving gaps underneath or between sections. No violation, immediate remediation is encouraged due to heavy storms predicted for the area.

>>Large amounts of accumulated clay dust on the access road between the area of the new construction and the existing pad may present a safety hazard when wet as well as increased risk of sedimentation in runoff.

>>At the time of inspection operator personnel reported initial cement work had just been completed prior to cellar installation.

2756447 Primary Facility 2018-05-24 Follow-up Inspection No Violations Noted On 5/24/18 a follow up inspection of the Kevech pad was conducted in relation to issues noted in a previous inspection. The following was observed:

>>Deficiencies in implementation of site BMPs had been addressed and satisfactorily remedied. No further concerns were found.

>>Construction of site accessory areas was ongoing at the time of inspection.

>>A section of redundant silt fence was noted to be constructed along an exposed fill slope above the access road, consultation with EQT's Jason Tyler indicated that the fence had been put in place to catch "clumpies". The fence did not change function or flow of water on site. No violation.

2757088 Primary Facility 2018-07-26 Drilling/Alteration No Violations Noted On 7/26/18 a routine complete inspection was conducted of the Kevech well site. The following was observed:

>>A large soil stockpile near the impoundment area of the site was lacking stabilization and appeared to be suffering minor erosion damage in the area used to access the pile. No controls directly down gradient from the access could allow runoff and material to enter the access road and leave the site. Remediation strongly encouraged.

>>Substantial sediment was found accumulated in the collection channel between the large soil stockpile and main access road. Any runoff would likely carry sediment into the pictured under road culvert and into the vegetated strip on the side of the road. Compromised CFS with excess sediment overtopping the control was found directly up gradient along the channel. It is likely the source of the the accumulated sediment. Remediation recommended.

>>Cleanup from drilling activities were ongoing at the time of inspection.

2757126 Primary Facility 2018-07-27 Follow-up Inspection No Violations Noted On 7/27/18 a follow up inspection of the Kevech well site was conducted in response to issues noted during a previous inspection. The following was observed:

>>The access point to the large soil stock pile was being stabilized at the time of inspection.

>>CFS between the stock pile and the access road had been cleaned and repaired with added surface matting added above it.

>>The production fluids remained in the containment unit but spilled fluids and contaminated pad materials had been removed.

2853243 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Well appears to be well maintained. Well is temporarily shut in due to low pressure.
