2679220 Primary Facility 2018-01-08 Routine/Complete Inspection No Violations Noted On Friday, January 5, 2018, I conducted a routine-complete inspection of the NV 78 CBM well located in Morris Township, Washington County.

The access road and the pad appeared to be in good condition and some vegetative growth was observed through the snow cover. The earthen berm secondary containment around the production tanks was observed to be partially vegetated and in decent condition.

The gas well and associated infrastructure appeared to be in good condition and the well was labelled.

No violations were identified during the inspection.

2754205 Primary Facility 2018-07-11 Routine/Complete Inspection No Violations Noted On Wednesday, July 11, 2018, I conducted a routine-complete inspection of the NV78 CBM well (125-23616) and surrounding site located in Morris Township, Washington County.

The access road and the pad area were observed to be in good condition.

The site has been stabilized with vegetation.

The earthen berm secondary containment structure appeared to be in good condition and functioning as designed. The berm was observed to be primarily vegetated with thinner sections of growth. The operator should consider addressing the berm to establish more uniform growth.

The production tanks also appeared to be in good condition.

The operator should consider utilizing secondary containment constructed with impermeable material. A release of fluid within the berm area can result in violation being issued to the operator.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

2833494 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted On Friday January 18, 2019 I conducted a routine complete inspection at the NV78 CBM Well Site located in Morris Township, Washington County.

The access road and the pad area were in good condition.

Uniform vegetative growth was observed through a thin coating of snow.

The earthen berm secondary containment around the production tanks appeared to be in good condition was observed to be vegetated. The production tanks also appeared to be in good condition.

Although the operator is compliant with current regulations, they should consider using secondary containment constructed of impermeable materials. A release of fluid inside the berm can result in a violation being issued to the operator.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
