The operator has had the service rig on site for 4 years which had no number or name on it. The well head was not tagged or have any sign identifying the facility. I sent our DEP plugging inspector the coordinates and he identified it as the TS Grier 402 (125-01515).

The cuttings tank on site was filled with rain water and discharging to the ground. The water inside had a sheen and was contaminated by the residue in the tank. There was a oil drum laying on the ground that was discharging crude oil. The fluids were all contained in the secondary containment berm onsite. Housekeeping was poor at the site. tools, buckets, barrels and pipes were cluttering the site.

4/26/17- I met with Mike Graham from New Century Energy to discuss the clean up and the other concerns at the site.

The release was less than 42 gallons and New Century Energy is going to remediate the site under DEP spill policy for site specific standards. Mike Graham was pro

Inspected site with Department WQS, land owner and operator. There is no identification at the well. Open tank with drill cuttings and fluid has been emptied and will be removed. Oil-saturated soil has been removed. Operator will improve dike around separation tank.

New Century Energy rig still on site. Rig has been on location for several years. Location is in bad condition with rusty metal, miscellaneous equipment on the ground and overgrown vegetation. At inspection on 4/26/17 operator stated rig would be moved within two (2) weeks and well identification would be in place. This location hazardous. Latitude and longitude at well head is 40¿14' 10.30" N; 80¿ 9' 24.10" W. Note: There is still no identification at this well.

2727020 Primary Facility 2018-05-07 Follow-up Inspection No Violations Noted New Century Energy rig remains on site. It appears some rig activity has taken place since last inspection. The rig has been at this location for several years. At a meeting at the site on 4/26/17 the operator stated improvements would be made to the site, including improving the dike on the separation tank, and the rig would be moved in approximately two (2) week. During an inspection and subsequent Notice of Violation (NOV) on 8/24/17 the rig remained on site and no improvements were made to the dike. Contaminated soil had been cleaned up and other improvements were made to the site. In a NOV response letter dated 10/10/17 the operated stated cement had to be removed and drill pipe fished out of the well bore and the well would resume production and equipment removed. This was not done.
2729006 Primary Facility 2018-05-10 Compliance Evaluation Outstanding Violations - Viols Req'd Followup inspection after Notice of Violation (NOV). New Century Energy rig remains on site. It appears some rig activity has taken place since last inspection. The rig has been on this location for several years with little activity. At a meeting on this site on 4/26/17 the operator informed the Department improvements would be made to this site, including improving the dike around the separation tank, and that the rig would be moved in approximately two (2) weeks. That did not occur. A NOV was issued to the operator due to failure to improve the site and remove equipment. In a NOV response letter dated 10/10/17 the operated stated pipe had to be fished from the well bore and the rig would remain to run tubing and rods to get the well pumping and then the rig would be removed. The operator has failed to do this.

Note: An error was made on a followup inspection dated 5/7/2018 (Inspection Record No. 2727020) where Inspection Results indicated "NOVIO - No Violations Noted". This site remains in viol
