2442245 Primary Facility 2016-01-20 Routine/Complete Inspection No Violations Noted Conventional oil well completed and appears operational. The location is frozen and snow covered. No apparent leaks or spills. Adjacent to the well is a tank battery consisting of 2-210 bbl stock tanks # 2978-3 and 2978-4 with a separator and a poly brine tank. There is adaquate containment. There were no apparent leaks or spills. No violations noted
2726261 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted This is a combination, oil and gas producing well. There is no identifying well tag affixed to the well or pump-jack. There is a storage facility in close proximity to the well. The facility consists of a separator and two, 210 bbl tanks, surrounded by one continuous diking. There are also two medium sized plastic brine tanks directly beside and slightly below these lager tanks. These plastic tanks are also surrounded by diking, however, there is a ditch dug in the lowest side of the diking. There are no spills or releases noted. There are no E &S related issues noted. An attempt was made to reach the operator via telephone to discuss the well tag and brine tank containment.
2781363 Primary Facility 2018-09-28 Routine/Complete Inspection No Violations Noted This inspection was conducted to document actions taken by the operator since this facility was last inspected.

An API number has been written on the pump-jack for identification. The trench in the brine tank containment does not appear to have been filled in. It is advised that this be corrected to avoid possible brine migration in the event of a spill.
