The well is combination oil/gas producing, fitted with an electrically driven pump jack. The API is clearly written on the pump-jack. This report was developed to document a meeting between my self and an equipment operator named Matt, who works for Vanderhoof Energy. The meeting was held to discuss the ongoing maintenance of the main access road for the cited location. The access road is very steep in some areas and seems to have been neglected in the way of maintenance for some time. The road is sunken, and has developed some significant erosional channels. I suggested the installation of several grade breaks/diversion channels in the steeper areas off the road. An artificial ditch line has developed on the cut bank side of the road. I suggested that this be removed to allow running water to reach the actual ditch line. I also suggest that general maintenance be continued on the road to avoid such a significant d

2822235 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted This location can be accessed off of Telic Run Rd. in Brokenstraw Township, Warren County PA.

This inspection was conducted to document the on going status of this facility and its accessing road, fallowing the past meeting between my self and an equipment operator named Matt, who works for Vanderhoof Energy. Since our meeting, two water bars have been installed on the access road. The water bars, however, are not conducting water to the ditch line and water continues to travel down the center of the road. There does not appear to have been any other work done. The need for more extensive road work was discussed with Matt, who was again on the lease at the time of this inspection. It was suggested that the road improvements be done, weather permitting, between now and spring time.

It was also noticed that one of the fittings between the buried gathering line and the production string for this well is leaking. A relatively small area around the well head has been covered with an oily emulsion. This leak s
