2844439 Primary Facility 2019-02-22 Routine/Complete Inspection No Violations Noted The main access road entrance for this well location is just off of Crescent Park Exd., in Mead Township, Warren County, PA.

This well was recently serviced and is powered by a rod line system. There are no significant leaks or spills noted. The operator assigned ID number is written on the jack. The operator has indicated that a well tag, with the appropriate API ID number will be placed on the well, soon. The well is positioned at the top of a long, and very steep, ATV trail. There are no E & S related BMP's in place to help stabilize the route, and there is the potential for extensive erosional activity. It was initially advised that the road be stabilized and abandoned. The trail, however, according to the operator, is the main accessing route for the well. It then was suggested that maintenance activities be implemented to help manage the route. The construction of several, "thank you ma'am's", at various places on the route, were discussed. It was also suggested that the route be used at a minimum.
