2752937 Primary Facility 2018-07-16 Routine/Complete Inspection No Violations Noted Weather was 75° and sunny at time of inspection. Met with Rockdale Marcellus representative Luke Chesner and Matthew Knight on location. Rig was in the process of moving onto this well to drill down to the kickoff point. They are estimating to drill this well to 5300' and set a retrievable plug. No violations were noted at time of inspection.

2763035 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted Routine Inspection; 8/1/2018.

I entered the site at 10:30 and inspected the stormwater controls along the access road. The area is well vegetated and temporary E&S controls are in the process of being removed. On the well pad, I talked to Mr. Carrasco (rig manager) from Patterson -UTI and we walked the site. At the time of the inspection, the BOP stack was installed on the Guillaume 6H (117-22003) but flow line was not yet connected. The derrick was up but cuttings management/backyard is still being rigged-up. Containment is in good condition and the wooden rig mats are also in good condition. Drilling additives and potential contaminants are stored in containment or have additional individual containment/cover. After walking the site, I talked to Danny Brooks (company man) and discussed the containment, maintenance, and the other two wells planned for the site. Overall, the site is in good condition. No violations noted.

2765049 Primary Facility 2018-08-20 Routine/Complete Inspection No Violations Noted Weather was 66° and cloudy at time of inspection. Met with Rockdale Marcellus representative Jim Tracey. Rig was in the process of drilling the 8 1/2" hole for the production casing. Rig expects to TD the well later this after noon and cement on 8/21/18. When this well is completed this rig will move to the Wesneski pad to complete those wells. Company man informed me of a spill that was reported at approximately 4:30pm on 8/19/2018. The main kelly line feeding the top drive sprung a leak and sprayed oil based mud across containment. A small amount went onto the ground and vegetation on the edge of location. Location was being cleaned by Elk Environmental Services at time of inspection. WQS (Water Quality Specialists) are expected to follow up. Conducted a walk around inspection of location. No violations noted at time of inspection.
2765932 Primary Facility 2018-08-20 Incident- Response to Accident or Event Violation(s) Noted Spill Response Inspection; 8/20/2018. At the time of the inspection, a Patterson UTI rig was drilling the production hole on the 2H well 117-22001). Rockdale Marcellus, LLC reported that the Kelly hose on the rig broke at about 16:40 on 8/19/2018. The hose failure caused approximately 30-35 gallons of drilling mud to be released. The spray of drilling mud left the containment spraying to the south/southwest.

I met onsite with Brett Neely and Jennifer Compton (both Rockdale) and we could easily see diesel-based drilling mud sprayed in the backyard/ cuttings management area of containment and down into the infiltration basin (see photo 6). An excavator was scraping the impacted soils just off the pad surface. In areas where the vegetation is thick, drilling mud did not penetrate to the ground. In the infiltration basin, the drilling mud was on the vegetation, bare ground, and floating on the water.

2810352 Primary Facility 2018-11-29 Routine/Complete Inspection No Violations Noted On 11/29/18 an inspection was conducted to assess site conditions and to check on the progress of the cleanup of a spill reported on 10/24/18 (CTS# 337795). No Rockdale reps were present upon arrival on location. At the time of the inspection, fracing operations had been completed & the wells were flowing back. A small battery of tanks on secondary containment were present near the well heads. The secondary containment from the frac had already been removed. A large aboveground storage tank (AST) is present on the NE corner of the pad. The tank is double-walled and plastic-lined. The SE corner of the pad was being excavated at the time of the inspection. Adjacent to this area of excavation were multiple roll-off containers containing excavated soil. In another area between the frac tank battery near the well heads & the AST, 2 more roll-off boxes were present. A separate area appeared to have been excavated adjacent to the AST on the NW side.
