2742616 Primary Facility 2018-06-18 Routine/Complete Inspection No Violations Noted The DEP conducted a ROUTINE inspection of the Seneca Resources – DCNR 007 Pad K wells site located in Delmar Twp., Tioga Co. on 6/18/2018 @1300 hours. The weather was 87 degrees, 64% humidity, winds W 11mph. Rain showers in the area. Gus Trejo, GT Drilling Consultants was present for the inspection.

Seneca is in the process of drilling 6 utica wells with the Patterson 339 rig. At the time of the inspection Haliburton was cementing the intermediate casing on the 123HU well. Mr. Trejo stated the rig will resume drilling of the production string tomorrow. A hose discharging “flop water” into a WM container broke free and spilled what appeared to be ~10-15 gallons of mud/water mix onto the rig mats on containment. The spill was immediately vacuumed up and the area appeared remediated before I departed the site.

No puddles were observed on the pad surface. The E&S controls appeared dry. No fluid was observed in secondary containment. The chemical storage area was observed in the NW corner of containment. Mr. Tr

2752419 Primary Facility 2018-07-16 Routine/Partial/Aerial Inspection No Violations Noted The DEP conducted a PARTIAL inspection of the Seneca Resources-DCNR 007 Pad K located in Delmar Twp, Tioga Co. on 7/16/2018 @1400 hours. The weather was 83 degrees, sunny, humidity 78%.

A Well site Restoration Extension Request(WSRER)was submitted on 6/26/2018. The filing requests a 24 month extension. The attached restoration plan shows a reduced pad size and the rain garden #7 infiltration basin. As of today's inspection, the pad size was observed at full size. The site is stabilized. Rain garden #7 was observed off the N side of the pad.

The onsite restoration drawings do not match the restoration drawings submitted with the WSRER. Please ensure only the most up-to-date plan drawings are located in the mailbox.

The WSRER is accepted at this time. I departed the site at 1445 hours.

2772509 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the DCNR 007 Pad K in Delmar Twp, Tioga County on 8/28/2018 at 1345 hours. The weather was clear, 74% humidity and 87 degrees.

Ellie Niles, DEP Water Quality Specialist, was onsite with me at the time of the inspection. The Patterson Rig#339 is currently onsite in the process of drilling the third of the six total wells. We spoke with Company man, Kevin Bradford, on the noted issues. We mentioned the torn containment in the chemical storage area, and the damp soil and puddles beneath the drill cuttings waste containers on the eastern side of the pad. He said the issues would be taken care of right away.

The following violations were cited against the ESCGP# ESX121170034(1): The waste containers on the eastern side of the pad not managed within secondary containment are considered in violation of Pa Code 25 Section 78a 64a(b)- All regulated substances including solid wastes must be managed within secondary containment on the well site and is cited at this time. Due

2785880 Primary Facility 2018-10-09 Drilling/Alteration No Violations Noted Not inspected due to drilling 116HU well.
2797669 Primary Facility 2018-11-02 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post plugging inspection for two conductors (115HU and 114HU) that were plugged earlier this week. At the time of inspection, CT on location for toe prep on other unconventional wells of the pad. I met with the Companyman who confirmed their plugging. The crew is pressure testing at the time of inspection. I did not approach the plugged wells at this time due to energized equipment. Plugged well monuments were observed from the edge of the pad. No obvious issues were observed.

2803677 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the Seneca Resources DCNR 007 Pad K in Delmar TWP, Tioga County on 11/14/2018 at 1200 hours. Ellie Niles, DEP WQS, accompanied me for the inspection.

Adam Pierce was onsite an accompanied us around the site. He confirmed that only 4 wells were drilled, that the current frac was on the 125HU and that the wireline services were working on the 124HU. They started the frac on 11/11/18. They are bringing water from impoundment #1 through the above ground waterline for use on the frac. "Select" is the company monitoring water transfer.

We found a couple puddles in front of the frac tank containment on the east side of the pad with elevated conductivities. (Photos #4 & #5) This area is where trucks have been offloading fluid from the frac tanks. Adam Pierce had the onsite vac truck driver suck up the fluids as soon as possible. The areas after remediation are seen in photos #8 & #9.

2822461 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Seneca Resource's DCNR 007 Pad K in Delmar Twp, Tioga County on 1/4/2019 at 1000 hours. I spoke with company man Andy and Guy Williams. Both of them accompanied me around the site.

Keane was currently working on the last stage of the 116HU frac. No more fresh water is being ran through the above ground waterline from the impoundment. After the frac is done (today) they will rig down and bring in another rig to drill out the plugs which plans on coming this Tuesday. The frac is moving to the DCNR 100M in Trout Run after this.

Site conditions looked overall good. A couple of the chemical storage secondary containments were packed down due to being driven over (see photo #2).It was mentioned that the equipment will be moved shortly due to rigging down and containment will be removed. Most fluids onsite were frozen. The area in front of the frac tank containment (was a mess last visit) is now straightened up and has containment laid down(see photo #3).
