At the time of the inspection, perimeter E&S controls were installed, trees were cut/removed and earthmoving had begun. Overall, the site is in good condition. No violations noted.

2769026 Primary Facility 2018-08-29 Preoperation Inspection No Violations Noted I met with Repsol Construction Foreman, Johnny Pazzaglia, on site. He explained that they installed a rock lined ditch (photo 4) in the channel that was planned to be vegetated, because a ground water spring is flowing water through it. Water in the ditch is 0.03mS/cm w an Oakton PCSTestr 35 today.

E&S controls are in place to comply w: A^§ 3258. (a.1) Preoperation inspections.--The operator may not commence drilling activities until the DEP has conducted an inspection of the unconventional well site after the installation of erosion and sediment control measures. JLP

2779771 Primary Facility 2018-09-26 Routine/Complete Inspection No Violations Noted Weather was 64° and raining at time of inspection. There were no Repsol representatives on location at time of inspection. Well is a spud conductor only. Well has a rectangular concrete and steel cellar installed. Conductor casing and the mouse hole were set at time of inspection. The cellar was covered with a steel grate. No violations were noted at time of inspection.
2806660 Primary Facility 2018-11-26 Incident- Response to Accident or Event Violation(s) Noted The department conducted an incident inspection of the Repsol Hughes E 02-204 well site in response to spill notification #338349. Also present for this inspection: Brad Kline, DEP water quality specialist, Eddie Tate, Patterson UTI, and Trent Anderson, company man for Repsol. On November 22, 2018, an estimated 30 bbls (1,259 gallons) of spud mud, or water-based drilling mud, was released from the top of the conductor pipe while the pipe was being set. The mud filled the well cellar and impacted soil beneath the secondary containment liner.

Eddie Tate and Trent Anderson informed us that immediately following the incident on the 22nd, rig mats were taken up and the secondary containment was cut in order to assess the extent of the impact to the surrounding soil. The same day, all drilling mud present was sucked out. Rig mats were replaced and secondary liner was repaired on November 25, the day before this inspection. 2824626 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted I met Trent Henderson of Repsol when I arrived. Patterson rig 278 on location running casing in the 6H well. The rig will be skidding to the 4H next. Overall condition of the containment appeared to be excellent, and the contained areas were quite clean and neat. The cuttings backyard was also very clean.
