Rig was in the process of waiting on cement at time of inspection. Halliburton completed the surface casing cement job at 4:19 am. Rig is estimating to begin drilling the 12 1/4" hole for the intermediate casing later in the afternoon. Performed a brief walk around inspection of location. No violations noted at time of inspection.
2721952 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted Weather was 50° and cloudy at time of inspection. Met with SWN representative Robert Gray on location. Halliburton completed the intermediate casing cement job at approximately 9:30 on 4/21/2017. Reviewed the cement job log and no discrepancies were noted at time of inspection. Rig was shut down at time of inspection for repairs on the flow line. Rig is drilling the 8 3/4" hole for the production casing. Rig is estimating to TD the 8 3/4" hole at 5850'. Once the rig reaches TD they will trip out of the hole and move off location for SDC45 to come finish the production hole. Performed a brief walk around inspection of location. No violations noted at time of inspection.
2725787 Primary Facility 2018-05-03 Routine/Complete Inspection No Violations Noted Weather was 63° and cloudy at time of inspection. Met with SWN representative Andrew Griffith on location. Rig was currently in the process of trying to fish out their BHA (bottom hole assembly) from the well bore. On 5/1/18 when they ran in the hole and began drilling they were seeing uncommon and sporadic pressures. They shutdown and tried to troubleshoot the problem. Upon coming online with the mud pumps they noticed a loss of approximately 700 psi and shutdown again to trip out of the hole. Their mud motor had parted and left most of their BHA at the bottom of the hole. They latched on once while fishing and began to retrieve the BHA only to have it come loose and fall back to bottom. During current inspection it was believed to be that they had successfully retrieved most of the BHA from the well bore. Mr. Griffith still intended on running a trash basket and a magnet run to bottom to ensure all of the pieces that may have come loose while fishing are retrieved. 2728799 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted Weather was 65° and sunny at time of inspection. Met with SWN representative Johnathan Woodburn on location. The hole for the production casing was TD'd on 5/8/18 at approximately 8:30am. Rig was preparing to run casing into the hole. They expect to cement at approximately 3am on 5/10/18. Performed a brief walk around inspection of location. No violations noted at time of inspection.
2728941 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted Weather was 55° and sunny at time of inspection. Met with SWN representative Johnathan Woodburn on location. Halliburton was on location pumping cement at time of inspection. Stayed and observed production cement job. Noted a leak on the accumulator unit for the BOP during the job. Absorbent pads were under the leak and the accumulator was elevated up on the lower level of the rig. There was no spill to the ground. Bumped plug at 11:00 am. Performed a walk around inspection of location. No violations noted at time of inspection.
2735349 Primary Facility 2018-05-24 Routine/Complete Inspection No Violations Noted Weather was 70° and sunny at time of inspection. There were no SWN representatives on location at time of inspection. Routine inspection of well that was recently cemented to TD. Well has not been stimulated at time of inspection. Cellar is bubbling at a constant rate. Unable to determine the source of the gas percolating up in the cellar at time of inspection. Using a plastic bottle I was able to concentrate some of the bubbles and test with an Altair 5X gas meter that was calibrated on 05/10/2018. 100% LEL and 10% methane was detected in the bottle of gas coming from the cellar at time of inspection. Will follow up with SWN to determine the source of the gas. No violations noted at time of inspection.

2746998 Primary Facility 2018-06-25 Routine/Complete Inspection Pending Weather was 74° and sunny at time of inspection. Routine inspection of well that was recently cemented to TD. Falcon Drilling flow back hand Kevin Miles was on location monitoring the annulus vents. At time of inspection 3 annulus vents were plumbed to surface. Tested the 3 annulus vents with an Altair 5x gas meter that was calibrated on 6/20/2018. The 13 3/8" x 20" annulus vent had 14% methane and 100% LEL. The 9 5/8" x 13 3/8" had 100% methane and 100% LEL. The flow back hand said they have been strapping the tank and the 5 1/2" x 9 5/8" annulus has not produced any water since the latest remedial work. Was unable to meter the annulus vent due to the plumbing. Will follow up with Afton Sterling for his weeks operations.
2750205 Primary Facility 2018-07-10 Routine/Complete Inspection Pending Weather was 85° and sunny at time of inspection. Met with SWN representative Ed Blystone on location. At time of inspection 3 annulus vents were plumbed to surface. Tested the 3 annulus vents with an Altair 5x gas meter that was calibrated on 6/20/2018. The 13 3/8" x 20" annulus vent had 20% methane and 100% LEL. The 9 5/8" x 13 3/8" had 100% methane and 100% LEL. The 9 5/8" x 5 1/2" annulus had 15% methane and 100% LEL. The plan that Ed had on location was to continue monitoring the annulus vents. Will follow up with Afton Sterling for this weeks operations.
2756210 Primary Facility 2018-07-24 Routine/Complete Inspection Violation(s) Noted Weather was 72° and raining. It had been raining for the last couple days. Met with SWN representative Ed Blystone on location. There was a work over rig on location. Rig was in the process of running a retainer into the 5 1/2" production casing at 2967' in preparation to squeeze the perforations at 2991'. Halliburton was on location to perform the squeeze. Tested the 20" x 13 3/8" annulus and 13 3/8" x 9 5/8" annulus. 20" x 13 3/8" annulus had 100% LEL and 15% methane detected and 13 3/8" x 9 5/8" annulus had 100% LEL and 100% methane detected at time of inspection. Gas percentages were confirmed by Ed Blystone. Circulation was established at 2 bpm and 2100 psi. At time of departure Halliburton was in the process of spotting cement in the tubing to squeeze the perforations. Will follow up with Ed Blystone to confirm squeeze operations outcome.

2759780 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted I met Ed Blystone of SWN when I arrived. CC Forbes workover rig was on location; Ed informed me that they were performing remedial work on the 2H well after discovering gas escaping from the 13" casing. The reported stimulation of the well had not occurred due to this issue. Containment was in good condition, and overall the site appeared clean.
2772484 Primary Facility 2018-09-04 Routine/Complete Inspection No Violations Noted Site restoration inspection with Sasha Minium and Mike Jurbala (DEP). A SWN employee was present at the entrance to the pad. Tom Chitwood Jr of SWN arrived at the site as well.

One of four permitted wells has been drilled but not yet stimulated.

The operator submitted a post-drill well site restoration report for this site which prompted this inspection. The site is stable and the vegetative cover overall is greater than 70% with the exception of the areas immediately above and below the newly installed rip rap on side slope of pad. A bioretention berm, along with a rock-lined ditch, provides PCSM. The site has been permitted to remain at full pad size. At this time, due the presence of a section of newly replanted vegetation which has not yet stabilized, as well as the presence of a frac tank, secondary containment, and other equipment not required for production, the site does not yet meet all of the post-drilling site restoration criteria under 78a.65. 2786386 Primary Facility 2018-07-26 Routine/Complete Inspection Outstanding Violations - Viols Req'd Weather was 72° and sunny at time of inspection. It had rained for several days before this inspection. Met with SWN representative Ed Blystone on location. Renegade was on location with a wireline truck to set a composite plug below the top perforations in preparation to squeeze cement. A second set of 6 holes will be shot to help ensure a successful cement squeeze on 7/27/18. Halliburton is expected to be on location tomorrow to perform the squeeze operations.
2786388 Primary Facility 2018-08-13 Routine/Complete Inspection Outstanding Violations - Viols Req'd Weather was 68° and raining at time of inspection. Met with SWN representative Ed Blystone on location. Rig was in the process of running in the hole with a 5 1/2" casing patch. Patch will be centered at 4079'. Renegade was on location with a wireline truck to run a gamma log to verify location of patch before setting. Due to rain and high humidity conditions, did not test annular vents with Altair 5x. Will follow up later in the week to check annular spaces for the presence of combustible gas.
2795632 Primary Facility 2018-10-29 Routine/Complete Inspection Outstanding Violations - Viols Req'd Weather was 45° and cloudy at time of inspection. There were no Southwestern representatives on location at time of inspection. Inspection being performed to determine if there was a presence of gas in the annulus vents. The 20x13 annulus vent had 0% combustible gas detected at time of inspection. The 13x9 and 9x5 annulus vents both had 100% LEL and 100% methane detected at time of inspection.
2822610 Primary Facility 2019-01-03 Site Restoration No Violations Noted A post drilling site restoration report was submitted to the Department on December 26, 2018 which prompted this inspection. One well has been drilled and stimulated. It was not staged for production at the time of this inspection and secondary containment was still present around the well head. One GPU and sand separator are present with secondary containment. These appear to be in good condition overall. One steel produced water tank is also present within steel containment with spray-in liner. The tank and containment are in good condition.

An area of erosion is present on the pad side slope on the west side of the pad (see photo #4). In another area on the east side of the pad, the vegetation on the berm above the rip rap has not yet reached 70% vegetative cover (see photo #5). The remainder of the area within the LOD is well-vegetated and stable.
