Met with SWN representative Andrew Griffith on location. While drilling the surface casing, fresh water was encountered. The amount and depths were as follows, approximately 20 bbl/hr at 325 ft. 200 lb/hr 390 ft, 240 Bbl/hr at 430 ft, 250 bbl/hr at 675'. Reviewed the cement job log from the surface casing and no discrepancies were noted. The rig was in the process of running intermediate casing in the hole. Cementing of the intermediate casing was anticipated to happen at approximately 12 pm. Conducted an inspection of the cellars on the other 2 wells on location and the area surrounding the rig. No violations were noted at time of inspection.

Rig was in the process of drilling the 8 3/4 inch hole for the production casing. Kick off point is estimated to be at 6338'. Reviewed the job log from the intermediate casing cement job and no discrepancies were noted. Discussed the drilling activities the rig was intending to complete in the upcoming week. No violations noted at time of inspection.

Rig was in the process of drilling the 8 1/2 inch hole for the production casing. Kick off point was at 6293' and the landing point was 7795'. Mud weight was 14.1 ppg. Discussed the drilling activities the rig was intending to complete in the upcoming week. No violations noted at time of inspection.
2698147 Primary Facility 2018-02-26 Routine/Complete Inspection No Violations Noted Weather was 40° and sunny at time of inspection. Met with SWN representative Johnathan Woodburn on location. Rig was in the process of preparing to move to the 2H well on the pad. Cementing of the production string on the 1H well was completed at 9:30 pm on 02/25/2018. The estimated top of cement was 3203'. The cement company had some mechanical issues during the cement job but cement density was maintained and they bumped the plug. Discussed the drilling activities the rig was intending to complete in the upcoming week on the 2H well. Drilling on the 2H was estimated to be commencing on 2/27/18. No violations noted at time of inspection.
2699542 Primary Facility 2018-02-28 Routine/Complete Inspection No Violations Noted I met Andrew Griffith, SWN company man, when I arrived. SWN 45 rig onsite, currently drilling the 2H well. Depth was 701', and Andrew informed me they would begin to run surface casing at 762'. Andrew accompanied me on the inspection. Containment was in good condition, with very little accumulated stormwater. Housekeeping was also good. Ramps run over the berm in traffic areas seem to be keeping the berms and containment from damage.

2716528 Primary Facility 2018-04-10 Routine/Complete Inspection No Violations Noted Weather was 33° and cloudy at time of inspection. No SWN representatives were on location at time of inspection. Rig has recently moved off location to the Connolly pad. Well is shut in and has 3 analog pressure gauges attached. Needle valves to pressure gauges were all closed at time of inspection. No combustible gas was detected in the cellar or in the area around the well head at time of inspection. No violations noted at time of inspection.
2725494 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted I met John Cave of SWN logistics when I arrived. Frac was active and pressured up. Both the sitewide and individual secondary containments appeared to be in good shape. Two tanker trailers were on their own containment near the sand boxes, but the containment had been twisted and folded; I recommended that this containment be addressed. After my inspection, I met SWN company man Donnie Torres. Donnie told me the frac started Friday (4/27), and they are zipper fracing the three wells. They are getting through 4 stages a day, but had trouble initially getting the wells to open up, and are only 8 stages in.
2775140 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted Southwestern Energy (SWN) submitted a well site restoration report to the Department for the SWN TI-01-Mase Monte Pad on 8/29/18.

Three SWN employees remained in their vehicles during the inspection.

Due to recent rainfall water puddles had formed on the pad. The conductivity of one of these puddles was 167.1 µS/cm.

Three Marcellus wells are staged for production (1H, 2H, & 3H) and properly tagged. There are also 3 GPUs, 1 produced water tank, and 2 batteries of flowback tanks onsite. GPUs, storage tanks, and wells were in secondary containment. Standing water in the produced water tank secondary storage had a conductivity of 152.6 µS/cm. Standing water in the flowback tank/well secondary containment had a conductivity of 52.3 µS/cm.

The first PCSM BMP is a rock sump infiltration area on the N edge of the pad; conductivity of water retained by the infiltration area was 92.7 µS/cm. The second PCSM BMP is a rock sump infiltration area on the S edge of the pad. At the time of inspection both PCSM BMPs app

2837869 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted Inspection conducted in response to a spill of produced water reported to the Department by the operator on February 6, 2019. The report from the operator stated that the spillI was estimated at 20 gallons and occurred on the access road to the pad. I was met on location by Dillion Otten, regulatory analyst for SWN, upon my arrival. He told me that they had already finished excavating the area impacted by the spill and all of the excavated material was present in a roll-off container that was present on the pad within secondary containment. He stated that the container was filled entirely with material from the excavation. He also stated that the excavation was completed visually and aided by a soil conductivity meter.
