2695122 Primary Facility 2018-02-15 Routine/Partial Inspection No Violations Noted I spoke with Wayne Nichols (SWEPI) on site who explained the liner is compiled of a 40 mil plastic sandwiched between 2 layers of felt. SWEPI SET 4 conductor casings for the Utica Shale wells that they plan to drill this March. One of the conductors is larger (20") to accept fiberoptic equipment. Air temp 62F. Inspection log in mailbox near production tank; last inspection listed on February 10, 2018. JLP
2701818 Primary Facility 2018-03-06 Routine/Partial Inspection No Violations Noted I spoke with Rick Smith (SWEPI) on site. He explained they are drilling the last of 4 Utica well topholes, and plan to be done in a day or two.

I got electrical conductivity readings with an Oakton PCSTestr 35. A puddle on the pad gave a 0.30 mS/cm reading. SWEPI inspection log is in a mailbox near the production tank; last inspection listed on March 4, 2018. Air temp 39F. JLP

2813514 Primary Facility 2018-12-11 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI LP's Cruttenden 846 Well Pad in Middlebury Twp, Tioga County on 12/11/2018 1415 hours. The weather was partly cloudy and 27 degrees.

There are currently 2 actively producing wells onsite with one production tank. The pony pad to the West is being used to store some piping and some farm equipment. A puddle on the pad surface (after ice was broken) had a field measured conductivity of 490us/cm. No erosional issues observed. The cut slope to the East is well vegetated and appears to be stabilized.

No violations noted today, I departed the site at 1450.
