The weather was 52 degrees and sunny. WD Smith is the construction contractor. Construction is scheduled to be complete by 5/20/2017. As of todays inspection, Silt sock has been installed around the perimeter of the disturbed area. The rock constructed entrance is almost complete. Ditches are in the process of being constructed. The water pad has achieved final grade and is being used as a staging area for construction equipment. A copy of the well permit, ESCGP and onsite inspection log was located in the mailbox. ID sign was observed at the entrance to the access road. No violations are noted at this time. Please contact Mathew Nuss to schedule a prespud inspection when necessary. I departed the site at 1130 hours.

commence drilling activities until the DEP has conducted an inspection of the unconventional well site after the installation of erosion and sediment control measures. The DEP may conduct follow-up inspections of well sites and related activities to determine compliance with this chapter.

I met with Douglas Eubanks (SWEPI OSR) on site, and got the following electrical conductivity readings (mS/cm) with an Oakton PCSTestr 35: basin--0.18, pad (photo 2)--4.0, drilling mud containment Or (>19.99), ditch along pad--0.15. Eubanks had his workers vac up the liquid on the pad (photos 2 & 4) and the liquid in the drilling mud containment (photo 5). Eubanks said they used brine while drilling through some of the formations, and are now using synthetic oil based mud. Eubanks is contacting Diamond T to have them inspect the 2 liners under the drilling mud containers.

2655369 Primary Facility 2017-10-31 Routine/Complete Inspection No Violations Noted I met with Joe Wunderlich when I arrived, and shortly after met with Brett McCoy, who walked the site with me. Ensign 162 rig was on location and was at 16,042' on the 26H well. Containments appeared to be in good condition, and for the most part the site was clean. I did note a very small dusting of a white, crystalline substance on the pad surface near the chemical containment area. Brett felt it was likely calcium chloride flake, and given the windy conditions, it had probably been blown out of the containment. Brett immediately called a hand over and instructed him to scrape up the product.
2657983 Primary Facility 2017-11-07 Routine/Partial Inspection No Violations Noted Jacob Huggard (SWEPI OSR) and I met on site. He said they finished cementing the 3 Utica wells and are now checking the Blow out Preventers. Huggard and I walked the perimeter of the pad, and went down to the basin (photo) taking electrical conductivity readings with an Oakton PCSTestr 35. The highest reading on the pad surface was 4.5 mS/cm (photo), and liquid on the nearby containment gave a reading of 10.1 mS/cm. Huggard is ordering a vacuum truck to remove that liquid.
2662856 Primary Facility 2017-11-20 Routine/Partial Inspection No Violations Noted The highest electrical conductivity reading I got on the pad was 1.9 mS/cm today.

I spoke with Matt Jentsch and Heath Mcafee from SWEPI on site. Ensign 162 is drilling 3 Utica wells on this pad. Mcafee said they will suck up the liquid on the pad that measured 1.9 mS/cm, and that they have an EC meter to check other pooled liquid.

2670583 Primary Facility 2017-12-13 Drilling/Alteration No Violations Noted I met with Josh Pritchard (SWEPI HSE) on site. They are hydraulically fracturing the 3 Utica Shale wells at this site. I did not get electrical conductivity field readings, because there was no standing water on site. Air temp: 14F. JLP
2692685 Primary Facility 2018-02-06 Routine/Partial Inspection No Violations Noted I spoke with Kevin Morgan on site (Flowback OSR), they are setting up to flowback the 3 Utica wells on site. We spoke about the area of the pad that was not frozen (photo), which gave electrical conductivity (EC) readings of "Or" (>19.99 mS/cm) with an Oakton PCSTestr 35 today. I then spoke by telephone with Patrick Bernethy (SWEPI Environmental) who reported elevated EC readings last week, and had samples collected to help determine the constituents that might be causing the elevated EC. Violations could be cited pending the lab report on liquid sample 0935-383 that I collected today.

2702062 Primary Facility 2018-03-06 Follow-up Inspection No Violations Noted I got electrical conductivity (EC) readings from liquid with an Oakton PCSTestr 35, and from solids with a Hanna Soil Test. Drainage in the north side ditch that drains into the basin gave a 1.8 mS/cm reading. Water in the basin near the access road gave EC reading of 0.30 mS/cm. Air temp 36F. Violations remain outstanding from a previous inspection (2693747). JLP

I took electrical conductivity (EC) with an Oakton PCSTestr 35. Violations from February 6, 2018 remain outstanding. Air temp 35F. JLP

2718414 Primary Facility 2018-04-11 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted an inspection of the SWEPI LP – My TB Investments 6076 Well Site located in Deerfield Twp., Tioga Co. on 4/11/2018 @1030 hours. The weather was 42 degrees and overcast with strong winds. Pat Bernethy, Shell Environmental Coordinator was present onsite. SWEPI is in the process of removing pad material contaminated by production fluids due to a failure of containment during the frac. The spill was reported on 1/30/2018. SWEPI reported not being able to immediately remediate the pad due to on-gong activities.

As of today’s inspection pad material affected by the spill showing elevated conductivity is being removed into roll-off containers for disposal at WM’s Phoenix Facility. Matt Bailey, AECOM was onsite taking soil conductivity readings to ensure the extent of the spill was removed. Mr. Bernethy stated approximately 27 containers of material will be sent to the Phoenix facility. A geotextile material was being laid out on the areas already remediated. Due

2744407 Primary Facility 2018-06-21 Follow-up Inspection Viol(s) Noted & Immediately Corrected The DEP inspected the SWEPI LP – My TB Investments 6076 Well Pad located in Deerfield Twp, Tioga Co. on 6/21/2018 @1230 hours. The weather was 78 degrees, 51% humidity and winds N 9mph. Kevin Morgan, SWEPI Flowback OSR was onsite during the inspection.

The pad is currently setup for flowback. A series of tanks and a gas buster were observed in containment along the western side of the well site. The following field measured conductivity readings were obtained using an Oakton Multiparameter meter: Temporary tank containment: 6.72 mS/cm Puddle off corner of containment: 4.6 mS/cm Puddle on pad between wellhead and containment: 5.17 mS/cm

The area beneath the edge of the tank containment was visibly damp. Mr. Morgan was informed of these findings onsite and a vac truck was dispatched to the site. The vac truck arrived onsite and removed the fluid during the course of the inspection. The onsite inspection log noted the site was last inspected on 6/16/2018 with “no new tank or E&S issues”. 2855555 Primary Facility 2019-03-19 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's My TB Investments 6076 Well Site in Deerfield Twp, Tioga County on 3/19/2019 at 1035 hours. The weather was 36 degrees and mostly sunny. The act 9 sign was observed at the site entrance. The E&S Plan was available for review in the mailbox at the entrance. The inspection log was found in the mailbox by the production tanks. Company personnel noted no issues on 3/16/2019.

There are currently 3 Utica wells onsite flowing back (24H, 25H, & 26H). The site is well vegetated where applicable. The areas not vegetated are stabilized with blankets and straw. Silt sock is present downslope where vegetation isn't present. All puddles on the pad surface were measured within normal conductivity ranges. Containment structures were intact and no leaks could be seen. PCSM BMPs are present where planned and working.

No violations noted today. I departed the site at 1135 hours.
