E&S controls on the well site are in need of repair, however no sediment was observed outside the LOD. Please see attached photos for details. This inspection meets the requirements of 58 Pa. C.S. 3258 (a.1) regarding a "pre-operational inspection". Please address any deficiencies noted, and contact me with any questions.

2631089 Primary Facility 2017-08-30 Incident- Response to Accident or Event No Violations Noted Bret McCoy (SWEPI) showed me where some moisture is apparently seeping out of the east slope (photo) of the pad. The liquid in the ditch below the seep gave an electrical conductivity (EC) reading of 0.28 mS/cm with an Oakton PCSTestr 35, indicating that it is not contaminated by ions (e.g. salts). However yesterday, Matt Bailey (Arcadis) reportedly (CTS 329341) found higher EC (4-8 mS/cm) in the moist area of the the east slope with a conductivity probe, and ~0.50 mS/cm in the liquid. Some vegetation seems stressed, and there is orange staining on that slope (photo).

Ensign 162 is drilling the final of three Utica wells planned for this pad. Diamond T arrived and are planning to remove the slope material that is moist and set up a system to drain and contain any liquid from that slope. The DEP requests periodic updates regarding the status of SWEPI's investigation regarding the source of the elevated conductivity readings.

2632913 Primary Facility 2017-09-06 Routine/Partial Inspection No Violations Noted I met with Bret McCoy (SWEPI) on site, and we got meter readings from liquid with a PCSTestr 35, and from soil with a Hanna Soil Test (photos). Diamond T removed soil from the pad slope last week, and installed containment (photo).

McCoy expects to move the drill rig off site by Saturday.

2652621 Primary Facility 2017-10-26 Follow-up Inspection No Violations Noted Tom Mcnerney (DEP PG) and I met with Matt Jentsch (SWEPI), who expects to be done fracturing and drilling out plugs on the 3 Utica Shale wells by around November 15, 2017. He said that is when the site will be characterized and remediated by Diamond T.

We got electrical conductivity readings from the ditch that previously (insp rec 2632903) gave elevated readings (mS/cm): water in ditch--1.2, soil in ditch--2.0. A puddle on the pad gave a 0.33 reading.

2656938 Primary Facility 2017-10-31 Routine/Complete Inspection No Violations Noted I met with Phil Tillet and Don Howard when I arrived on-site. US Well Services Clean Fleet was on location performing the frac. Phil informed me that they expect to complete the frac within the next 24 hours. Don walked the site with me. Near the data van, there was a small (1-2 gallons) puddle on the pad surface that showed slightly elevated conductance (2.35 mS/cm); a much larger puddle immediately next to it showed low conductance (.65 mS/cm). Containments appeared to be in good condition. On the southwest side of the pad, I noted another small puddle outside of containment (1-2 gallons) that also showed elevated conductance (5.84 mS/cm); no other water was present in this area. Don told me he would have these puddles vacced. A water transfer pad was located next to the main pad, and was quite busy with water truck traffic. Puddles in the offloading area showed low conductance (.54-.71 mS/cm). Overall the site appeared clean and reasonably well-managed.
2662871 Primary Facility 2017-11-20 Follow-up Inspection No Violations Noted I spoke with Chris Underwood (SWEPI) on site. He expects to start flowback tomorrow.

I got electrical conductivity (EC) with an Oakton PCSTestr 35 from the ditch on the east side of the pad at 0.38 mS/cm, and EC of 0.42 from a puddle on the pad. JLP

2704997 Primary Facility 2018-03-13 Routine/Partial Inspection No Violations Noted I met Chris Underwood (SWEPI) on site today. The 3 Utica Shale wells are "flowing back".

I got electrical conductivity readings with an Oakton PCSTestr 35. A puddle on the pad gave a 0.37 mS/cm reading. Air temp 28F. SWEPI mailbox near production tanks holds inspection log; it lists their last inspection on March 10, 2018. JLP

2719300 Primary Facility 2018-04-11 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the Lingle 1102 Well Site located in Deerfield Twp, Tioga Co. on 4/11/2018 @ 1200 hours. The weather was 42 degrees and overcast with strong gusting winds. SWEPI personnel was onsite working on the production equipment.

There are three Utica wells onsite, the 21H, 23H and 25H. The wells are connected to a gas buster located on the same secondary containment as the wheelie tanks. Erosion was observed above the channel on the north side of the pad (This was also noted in the E&S inspection log). Water was observed in the ditches surrounding the pad. Puddles on the pad surface were tested for conductivity and found to be in normal range. The E&S and tank inspection were conducted by company personnel on 4/7/2018.

No violations are noted at this time. I departed the site at 1230 hours.

2811906 Primary Facility 2018-12-07 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI LP's Lingle 1102 Well Pad in Deerfield Twp, Tioga County on 12/7/2018 at 1130 hours. The weather was 23 degrees and snow flurries.

There are currently 3 active Utica wells on this site, the 21H, 23H, and 25H. They currently appear to be flowing back with three separators onsite. Another well was recently plugged on this site a few months ago. There are 7 frac tanks onsite, one appears to be a gas buster. All are within proper containment. All fluids observed on the pad surface and in containments were frozen and pad surface snow covered.

No issues or violations noted today, I departed the site at 1150 hours.
