Well was shut in and covered by a large orange steel cage at time of inspection. There were no,pressure gauges attached to the well at time of inspection. There was one annulus vent plumbed to surface that was shut in at time of inspection. No violations noted at time of inspection.
2725584 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted No activity on the pad when I arrived. Pipeline work at entrance and across the road. Containment was partially dismantled, with some of the rig mats removed and stacked, as well as some berms removed and stacked. Puddles on the well pad showed low conductance (.9 -1.6 mS/cm). The 8H well cellar was nearly full to the top with water; I recommend that it be vacced ASAP. Site looked good overall.
2738666 Primary Facility 2018-06-05 Routine/Complete Inspection No Violations Noted I met Will Stevens of SWN in the command trailer when I arrived. Frac was active (Halliburton), although it was pressured down for a move from the 7H to the 8H well. I was joined on my inspection by Jeremy Eichenberger and Allen Green of SWN. The site and sitewide containment were very clean, with no standing water. I did note in 3 locations (near the sand boxes, near the production tanks, and near the GPUs) where the sitewide liner had fallen or been blown off of the Jersey barriers forming the berms, and pointed these out to Jeremy and Allen. The issue seemed to be that the liner had not been installed long enough to reach the ground on the backside of the Jersey barriers; Jeremy and Allen proposed seaming an additional piece of liner to the existing liner to allow the liner to reach the ground so it could be secured with sandbags to keep it in place.
2773340 Primary Facility 2018-09-04 Routine/Complete Inspection No Violations Noted Southwestern Energy (SWN) submitted a well site restoration report to the Department for SWN TI-14-Connolly A Pad on 8/22/18.

Department staff Mike Jurbala, Sara Metz, and Sasha Minium conducted a site restoration inspection at the above site on 9/4/18. Two SWN employees remained in their vehicles during the inspection.

There are 3 Marcellus wells staged for production on the pad (Connolly A 2H, 7H, &8H). The wells were in secondary containment and the well cellars were covered by the containment. There are 3 GPUs and 1 production water tank within secondary containment.

The site meets 70% uniform, perennial, vegetative cover. All PCSMs have been installed as depicted on PCSM/SR plans. However, at the time of inspection flowback water tanks remained on site. As per 78a.65(a)(1) these tanks, their associated secondary containment, the secondary containment around the wells and any other equipment or containment not needed for production should be removed as part of the site restoration plan.

2811135 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Weather was 30° and cloudy at time of inspection. There were no Southwestern representatives on location at time of inspection. Well had digital pressure monitoring equipment on the production tubing. Production tubing pressure was 349 psi at time of inspection. There was one annulus vent plumbed to surface. It was plumbed into the pay line. 0% combustible gas was detected in any area surrounding the wellhead at time of inspection. No violations were noted at time of inspection.
