2482156 Primary Facility 2016-05-09 Preoperation Inspection No Violations Noted The Department of Environmental Protection (DEP) attended a preconstruction meeting at the SWEPI LP – Fuller 826 (ESX29-117-15-0006) located in Middlebury Twp., Tioga Co on 5/9/2016 @ 1100 hours. The weather was 55 degrees and sunny, winds WSW 5 mph. Present for the meeting were Wayne Swinehart, Civil Supervisor SWEPI LP; Joe Leonard, WD Smith Construction; Scott Lawrence, SWEPI LP. WD Smith stated they will begin construction of the site the middle of the next week. A revision was made to move the access road to an alternate location to avoid a landowner issue. The LOD of the access road and pad areas was walked. The tank pad is slated to remain after site restoration for use as a compressor station. Please ensure all temporary E&S controls are installed before earth disturbance activities begin. No violations are noted at this time. I departed the site @ 1200 hours.
2500493 Primary Facility 2016-07-06 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Fuller 826 well pad (ESX29-117-15-0006) located in Middlebury Twp., Tioga Co. on 7/6/2016 @ 1300 hours. The weather was 80° and sunny. An adequate site ID sign was observed at the entrance to the access road. A copy of the ESCGP-2 and well permit was located in the mailbox. The E&S inspection log shows the site as last being inspected on 6/15/2016 with “construction” noted in the comments.

The Fuller site is currently being constructed by W&D Smith. The access road is completed, but cap rock will be removed to soil cement the base for added stability. ~30% vegetative cover was observed along the access road in the ECB and TRM channels. Please ensure all matting has good contact with the ground to provide optimum erosion control. Cut and fill of the main pad and tank pad are scheduled to be completed by Friday. 2507484 Primary Facility 2016-08-02 Preoperation Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Fuller 826 well pad (ESX29-117-15-0006) located in Middlebury Twp., Tioga Co. on 8/2/2016 @ 1030 hours. The weather was 70° and sunny. An adequate site ID sign was observed at the entrance to the access road. Copies of the ESCGP-2 and well permits were located in the mailbox. The E&S inspection log shows the site as last being inspected on 7/28/2016 with “E&S working” noted in the comments.

A pre-operational inspection is being conducted at the request of Scott Livermore, SWEPI OSR. At the time of the inspection, the ECB lined channel along the access road, from station 7+00 to 2+00 had been excavated to allow installation of under-drain. The onsite contractor stated water underneath the road has become an issue so some E&S controls were being modified. Additional culverts were also to be installed along the access road to channel water from the under-rain to the infiltration basin. Silt sock was observed where specified on the E&S

2510887 Primary Facility 2016-08-15 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the Fuller 826N site (ESX29-117-15-0006) located in Middlebury Twp., Tioga Co. on 8/15/2016 @ 1300 hours. Jim Townsley, SWEPI LP and I were onsite for the Fuller 826 Spur Pipeline preconstruction meeting, when severely eroded channels and multiple failed BMPs were observed and documented. Please see attached pictures for photo documentation of the following E&S issues:

1) An eroded trench, from accelerated stormwater runoff, was observed in the bottom of roadside channel CC2 from ~stations 8+00 to 5+00.

    a. Erosion has occurred on the embankment above the roadside channel CC2 from ~stations 7+00 to 5+00. The Erosion Control Blankets (ECB) failed causing sediment to erode and deposit into the channel bottom.

2) Channel CC9 appears to have experienced gully erosion. The ECB has washed down into sediment trap 1B. 3) The slope protection on the southeast and west corners of the main pad, and on the SW corner of the tank pad, eroded sending rock and

2527339 Primary Facility 2016-10-11 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Fuller 826 well pad (ESX29-117-15-0006) located in Middlebury Twp., Tioga Co. on 8/11/2016 @ 1430 hours. The weather was 50° and sunny. An adequate site ID sign was observed at the entrance to the access road. Copies of the ESCGP-2 and well permits were located in the mailbox. The E&S inspection log shows the site as last being inspected on 10/7/2016 with “no rainfall last few days. No standing water on location. No noticeable erosion. BM15 moving off location and will be gone by end of day.” noted in the comments.

During the inspection Precision Pipeline was in the process of constructing the Fuller 826 pipeline spur. Parts of the pad were inaccessible due to welding and large equipment activities. The repaired E&S controls appear to be functional. Some areas are growing ~25% vegetative cover. A large amount of deposited sediment was observed in the sediment traps below the fill slope. 2546365 Primary Facility 2016-12-19 Follow-up Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Fuller 826 well pad (ESX29-117-15-0006) located in Middlebury Twp., Tioga Co. on 12/19/2016 @ 1230 hours. The weather was 20° and sunny. An adequate site ID sign was observed at the entrance to the access road. Copies of the ESCGP-2 and well permits were located in the mailbox. The E&S inspection log shows the site as last being inspected on 12/18/2016 with “No erosion noticed. Snow, ice and water on pad.” noted in the comments. A followup inspection is being conducted to check the status of violations issued 8/15/2016 for accelerated erosion, BMP failure and failure to conduct post-stormwater inspections as specified in the permit.

As of today’s inspection the Ensign 162 rig is onsite drilling the 26H well. The rig is currently at a depth of 11603’. The rig previously drilled the 22H well to a total depth of 19409’.
