2509256 Primary Facility 2016-08-10 Routine/Complete Inspection No Violations Noted This inspection was conducted to assess current site conditions. No Southwestern representatives were present.

Construction of the well site is still underway. The pad area has been cleared of timber, and temporary erosion and sediment control BMPs have been installed along the perimeter of the site. The areas disturbed by access road construction are now vegetated and appear stable. No evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

2518671 Primary Facility 2016-09-13 Routine/Complete Inspection No Violations Noted nspection of perimeter erosion and sediment control BMPs conducted at the request of Southwestern Energy. Rich Gulyas and Tom Chitwood of Southwestern were present.

Installed perimeter controls consisted of a mulch berm along the entire perimeter, with compost filter socks installed as secondary controls in several locations. The site has been cleared of vegetation, but no major earth disturbance has begun. No evidence of accelerated erosion or sedimenation was observed. Southwestern indicated that construction and major earth disturbance on the well pad portion of the facility (access road already constructed) will commence on after October 1.

No violations were cited during this inspection.

2532064 Primary Facility 2016-10-27 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions and progress of construction. Recent heavy rains (4-6 inches in about 6 hours) occurred one week ago, and it was raining steadily at the time of inspection.

The pad areas has been graded and storm water basins constructed. Stone has not yet been placed on the pad surface. No evidence of accelerated erosion or sedimentation was observed. The perimeter erosion control on this site consists of a contiguous mulch berm with additional berms and filter socks in place at several locations. No sediment was observed to be leaving the limit-of-disturbance for the project.

2653612 Primary Facility 2017-10-27 Routine/Complete Inspection No Violations Noted I met Jonathan Woodburn on-site. SWN rig # 45 running production casing in the 5H well. Cuttings processing was idle. Containments appeared to be in good condition. The site was somewhat muddy, and Woody told me he had a vac truck scheduled to clean things up.
2665401 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted Inspection to assess site conditions after drilling operations. Preparations for well stimulation are underway. Containment has been placed on the pad surface, and crane mats are stockpiled on site. No issues identified and no violations cited.
2725508 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted I met Joe Forker, SWN company man when I arrived. A workover rig was on the site, and had been drilling out plugs on the 1H well; when I arrived, the rig was tripping out of the well. Joe informed me that they would be deploying fishing tools into the well to recover some live perf guns that were lost in the 8th stage. Joe walked the site with me. A 50-70 gallon puddle on the pad in a high traffic area had slightly elevated conductivity 2.36 mS/cm); all other puddles on the pad were significantly lower (.3- .9 mS/cm). I recommended to Joe that he have it vacced, and while I was still on the site Joe directed a vac truck driver to move to the puddle and clean it up. The containment and berms seemed to be in good condition.
2773050 Primary Facility 2018-09-04 Site Restoration No Violations Noted Site restoration inspection with Sara Metz and Mike Jurbala (DEP). A SWN employee was leaving the site as we arrived and requested that we lock the gate when we were finished.

Two of five permitted wells (Bliss 5H and 1H) have been drilled and stimulated. At the time of the inspection, it appeared that the wells were either still in the process of or had recently finished being flowed back.

This inspection was conducted in response to submittal of a post-drilling well site restoration report. The site is stable with vegetative cover exceeding 70%. PCSM has been addressed through installation of a bioretention basin that appears to be functioning as designed. The site has been permitted to remain at full pad size. At this time, due to the presence of the temporary storage tank battery, the associated tank battery secondary containment, and the secondary containment around the wells themselves, the site does not yet meet all of the post-drilling site restoration criteria under 78a.65. I recommend returning t

2842743 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted Weather was 40 degrees and sunny. Met with Southwestern representative Andrew Griffith. Well was in shut in and had its surface discharge manifold equipment removed. Well was covered with an orange steel cage due to drilling activities on location. Well had gas monitoring equipment to detect any presence of combustable gas during drilling activities. Well had one annulus vent (9x5) plumbed to surface and closed in at time of inspection. No violations were noted at time of equipment.
2845188 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted SDC 44 rig on location. I met Andrew Griffith of SWN on the site; rig was running casing in the 4H well, and Andrew told me the rig would be skidding to the 6H well on the evening of 2/26. Sitewide and secondary containments and berms appeared to be in good condition, and the cuttings processing area and backyard were clean and well maintained. Chemical storage areas were clean as well. Andrew informed me that they were using dual-fuel air packs which are capable of diesel and natural gas blended up to 70% diesel. Most of the air packs were not in use while I was on the site.
