2556380 Primary Facility 2017-01-25 Routine/Partial Inspection No Violations Noted Routine partial inspection of well pad and former impoundment area. Routine monitoring not completed. Well pad being prepared for arrival of drill rig. Six new wells have been permitted and were spud on 1/9/17. The 5/31/12 and 6/11/12 violations associated with the impoundment remain outstanding against the Phoenix 590934 well (117-21148). The 8/9/12 and 9/26/12 violations also remain outstanding against the Phoenix 590934 well. TLT
2601431 Primary Facility 2017-06-06 Routine/Complete Inspection No Violations Noted I arrived on-site @ 09:35 hrs. and met Bill Wallace of EQT. Patterson UTI rig 294 on location tripping into the 593851 well. Some housekeeping issues noted, all in containment and mostly related to precipitation; Mr. Wallace informed me that a vac truck was en route. I noted 2 spots where the primary containment may have been allowing rainwater through to the pad; Mr. Wallace was aware of one of the spots, and will have both checked and/ or repaired. I departed the site @ 10:20 hrs.
2613927 Primary Facility 2017-07-06 Routine/Complete Inspection No Violations Noted I arrived on-site and met Bill Wallace, EQT company man. Patterson UTI rig #294 on location. Bill informed me that they had just started drilling on the 593853 well ( permit #117-21786). He also told me that after my last inspection in which I identified some spots where the pad containment appeared to be leaking that they had wound up moving the rig off the site, pulling up the rig mats, and replacing the pad-wide containment due to issues with the liner system. The new liner was in good shape and appeared to be a heavier, thicker material. The pad was in good condition overall, with no issues noted.
2650856 Primary Facility 2017-10-18 Routine/Partial Inspection No Violations Noted On 10/18/17 at 0837 hrs I conducted an inspection at the Phoenix S site. It was sunny and 41 degrees F. EQT OSR, Chip Miller, accompanied me during the well pad inspection.

I observed the well pad and water storage pad; the former Phoenix S Pit was not inspected. The Phoenix 593850-593855 wells were drilled Jan.-July 2017; stimulation notices were received in September. EQT was drilling out plugs on the 593851 well and flowing back the 593850 and 593852 wells. Well pad containment consists of bermed, pad-wide containment with mats on top. Localized containment structures were observed under individual pieces of equipment. Standing water in the pad-wide containment was screened in the following locations for specific conductance: SE pad corner: 672 microsiemens/cm NE corner: 10.16 mS/cm NW corner: 7.04 mS/cm SW corner: 920 microsiemens/cm A tank farm (500 bbl tanks) was staged along the north side of the pad and held produced water from flowing back the wells. The tank farm was on mats in the pad-wide co

2659523 Primary Facility 2017-11-12 Incident- Response to Accident or Event Pending During the fracking process on stage 21 of 32 a well control incident took place, as water and combustible gas started to blow from the

20 x13 3/8 annulus vent (Conductor x Surface). Upon my arrival wild well control was on site and that that Wild well control was in control of the well pad. At 16:31 the plan of action was to rig down wireline and frack equipment to make room for coil tubing rig so that temporary plugging can begin on the well and other surround wells in close proximity. Plugging operations are planned to begin in the afternoon or late afternoon on 11/13/2017. At 17:15 i spoke with Jim Hermick (EQT) I was informed that flow has decreased significantly since the time incident took place. I was also informed that combustible gas has decreased to 10% LEL at the well head. Recommend water quality specialist to follow up to ensure that fluids have not left the well pad containment.

2659579 Primary Facility 2017-11-12 Incident- Response to Accident or Event No Violations Noted Rich Herbstritt (DEP, OGI) and I inspected this site after EQT reported loss of well control on one of the three Marcellus wells FTS (contractor) was hydraulically fracturing ("zipperfrack"). We spoke with EQT's VP of drilling/ completions on site, Jim Helmick, who explained the following: last night at ~2330 hrs, the Cactus (brandname) well head began to leak flowback and methane while FTS was fracturing. Between then and noon today, vacuum trucks have been removing the flowback liquid and delivering it to 3 locations: Hydrorecovery in Antrim, PA, a Seneca Resources site, and to an Above-ground Storage Tank (AST). They estimate vac trucks removed 1,900 barrels between 2330 and noon, and that the well is still releasing ~110 barrels/hr (~1 vac truck load/hr) into the cellar. Vac trucks have been able to keep up with removal of liquid from the cellar, preventing overflows. Wild Well Control out of Texas has personnel on site who are addressing the well control issue. Wild Well reportedly got an LEL re
2660805 Primary Facility 2017-11-15 Incident- Response to Accident or Event No Violations Noted On 11/11/17 at 11:38 a.m., during fracking operation, stage 21/32, produced fluid started to accumulate in cellar. A plug was installed in the well, which slowed flow but did not stop. The produce fluid had not left containment and trucks were able to keep up with flow; the nearest waterway was reported to be 2900' to the east.

On 11/15/17 at 10:20 a.m., an investigation was conducted at a distance no closer than 1,420 ft from the east and 2,016 ft from the south of the well pad. The Department of Environmental Protection determined this to be a safe distance to conduct the investigation based on the present conditions at the well site. No methane was discovered at ground level in the area east and south of the well site. An Altair V gas meter ( calibrated on 11/15/17 at 0830) was used at the time of inspection. Five SAC 046 samples were collected at this time (see photos and sample log for conditions and locations).

2662539 Primary Facility 2017-11-15 Incident- Response to Accident or Event Pending Well control incident during fracking stage 21 of 32. Fluid and combustible gas emitting from a 4" ball valve on the wellhead assembly. Plug attempt ongoing during duration of the day. No obstructions up to this point. No changes to note regarding gas or fluid flows from well. Attempt to set first plug in this well at approx. 6:00pm. No further progress noted while at location. Left location at 7:00 pm.
2666168 Primary Facility 2017-11-29 Follow-up Inspection Pending This inspection is a follow up to a well control incident. Wild Well Control no longer in control of this well. EQT regained control of this well on 11-28-17. At the time of this inspection well area was being prepped and backfilled for cementing around a new well cellar box. I went to well sight for a physical observation of ongoing prep work for this process. Cement trucks started to arrive at 10:30am and continued to come to well sight to pour cement until 1:10pm. Cement portion of this process was complete at this time. Well site area and pad prep for backfilling and pad grading and leveling continued through out the rest of the time i was present. I left this site at 4:00pm.
2666175 Primary Facility 2017-11-30 Follow-up Inspection Pending At the time of this inspection prep work for this well site and pad area were ongoing to prep area for top hole rig up at this well site. This work continued all day with pad backfilling , grading, leveling, new containment installing and rig mats being put in place. When i left this well pad prep work was ongoing.
