Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2397439 Primary Facility 2015-08-11 Routine/Complete Inspection No Violations Noted
2426092 Primary Facility 2015-11-17 Incident- Response to Accident or Event No Violations Noted Incident Inspection. SWEPI notified the Department on 11/16/15 that an estimated 5 barrels of stimulation fluid was released during frac operations. The force of the release tore the liner containment, allowing approximately half of the fluid to migrate onto the ground. Fluid was observed on the ground along the south side of the containment. Additional puddles were observed outside of containment on the north and east sides of the pad. Violations cited against the 23H well (117-21755). TLT
2439479 Primary Facility 2016-01-08 Follow-up Inspection No Violations Noted Follow up inspection. Spill that occurred on 11/17/15 has been cleaned up. SWEPI monitored rock-lined channels and other stormwater BMPs from 11/17/15 - 11/23/15. Monitoring confirmed impact limited to pad surface. Affected pad material excavated by 11/25/15. Twelve soil confirmation samples collected. SWEPI informed DEP on 11/30/15 of their intent to use the Alternative Remediation Method for spill cleanup documentation. Violations remain outstanding against the Miller 394 23H (117-21755) and will be resolved upon receipt and review of the Remedial Action Completion Report. TLT
2472044 Primary Facility 2016-04-06 Follow-up Inspection No Violations Noted Follow up Inspection. DEP received SWEPI's Remedial Action Completion Report on 3/18/16. The report documents the cleanup following the 11/16/15 release. The report was approved on 3/29/16. No issues were noted at the site. The excavated area is backfilled and stable. The 11/17/15 violations against the 23H well have been resolved. TLT
2513995 Primary Facility 2016-08-22 Incident- Response to Accident or Event No Violations Noted Inspection to follow up on a small spill reported on 8/15/16. Approximately 15 gallons of triethylene glycol was release to the pad surface from a tote with a faulty ball valve. Free liquid immediately soaked up with absorbent pads and effected soil scraped up for disposal. No evidence of staining on pad surface. One violation cited against the 23H well (117-21755) however, violation immediately corrected based upon prompt cleanup of spill. TLT
2587082 Primary Facility 2017-04-26 Routine/Partial Inspection No Violations Noted The puddles that were on the pad service were screened and no elevated conductivity was noted. The majority of site is stabilized. No violations were noted during this inspection
2605923 Primary Facility 2017-06-15 Site Restoration No Violations Noted Restoration inspection performed after receipt of the Well Site Restoration Report (WSRR) and ESCGP Notice of Termination (NOT) on 6/6/17. Interim site restoration completed in August 2016. All post construction stormwater management (PCSM) BMPs have been installed. Areas disturbed during restoration activities are permanently stabilized and temporary E&S controls have been removed. Based on existing site conditions, approval of the NOT is recommended. The WSRR was acknowledged on 6/19/17. TLT
2657866 Primary Facility 2017-11-07 Routine/Complete Inspection No Violations Noted Cold/snowing at time of inspection.

Well has wrong well tag with wrong API number

API for this well should be 117-21756

2688157 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted Cold/ windy at time of inspection. Well tag has been corrected since last inspection.
2688420 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the SWEPI LP Miller 394 well pad located in Delmar Township, Tioga County on January 30, 2018 at 1345 hrs. The weather was 20 degrees and sunny with 3-4 inches fresh snow on the ground. There was no activity onsite or company personnel during my inspection.

The 1Vwell was drilled in 2010 and placed on in active status. The 1H, 3H, 5H, 7H were spud, but not drilled to total depth. The 23H and 25H wells are currently in production.

PCSM BMPs were observed onsite. No violations or issues were noted. I completed the inspection at 1445 hours.

2829347 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Miller 394 Well Pad in Delmar Twp, Tioga County on 1/18/2019 at 1110 hours. The weather was 30 degrees, partly cloudy and 1/4 inch of snow covered the pad.

There are currently 2 wells onsite that appear capable of production, the 23H and 25H. They were both tagged with the appropriate API numbers. The well site remains wells vegetated and stabilized. No erosional issues were observed. It appears that a frac tank was just placed on pad this morning. It sits within proper containment.

No violations noted today, I departed the site at 1135.
