2471235 Primary Facility 2016-03-29 Administrative/File Review No Violations Noted SWEPI LP contracted Moody and Associates, inc (Moody) to monitor the storm water drainage system on the well pad from November 17, 2015 through November 23, 2015. Impacts were not detected beyond the gravel surface of the well pad. Excavation of impacted gravel on the well pad was completed on November 25, 2015. The excavated soil will be disposed of at an approved landfill once approval is received. Following remediation, twelve (12) soil samples were collected for laboratory analysis to confirm the effectiveness of remediation. Attainment samples were collected in general accordance with Title 25 § 250.703 General Attainment Requirements for Soil and analyzed in general accordance with the PADEP memorandum: Program Clarification Common Constituents for Oil and Gas Related Spills and Releases, dated October 3, 2014. A remedial action completion report summarizing remedial activities, analytical confirmation results and waste documentation dated March 18, 2016 was reviewed by thr DEP and no technical deficie

2688418 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the SWEPI LP Miller 394 well pad located in Delmar Township, Tioga County on January 30, 2018 at 1345 hrs. The weather was 20 degrees and sunny with 3-4 inches fresh snow on the ground. There was no activity onsite or company personnel during my inspection.

The 1Vwell was drilled in 2010 and placed on in active status. The 1H, 3H, 5H, 7H were spud, but not drilled to total depth. The 23H and 25H wells are currently in production.

PCSM BMPs were observed onsite. No violations or issues were noted. I completed the inspection at 1445 hours.

2829344 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Miller 394 Well Pad in Delmar Twp, Tioga County on 1/18/2019 at 1110 hours. The weather was 30 degrees, partly cloudy and 1/4 inch of snow covered the pad.

There are currently 2 wells onsite that appear capable of production, the 23H and 25H. They were both tagged with the appropriate API numbers. The well site remains wells vegetated and stabilized. No erosional issues were observed. It appears that a frac tank was just placed on pad this morning. It sits within proper containment.

No violations noted today, I departed the site at 1135.
