2442420 Primary Facility 2016-01-21 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected The Department inspected the SWEPI LP site, the Synnestvedt 878 (ESX10-117-0154) located in Osceola Twp., Tioga Co. on 1/21/2016 @ 1145 hours in response to a spill notification from Rory Hunter, Env. Coordinator for SWEPI LP, received on 1/21/2016 @ 0930 hours. The weather was 27° and cloudy. Present were myself; Rory Hunter, Env. Coordinator SWEPI LP; Mark Hoover, Production Supervisor SWEPI LP; and Rick Dake, Staff Associate SWEPI LP. At the time of this inspection, SWEPI had attempted to recover all visibly affected material possible with hand tools, placing it in barrels onsite. A vacuum truck was in route to remove the fluid remaining in the primary containment. A violation of 35 PS § 6018.301 of the Solid Waste Management Act is noted at this time for failure to properly store, transport, process or dispose of a residual waste. This violation has been immediately corrected. I departed the site @ 1245 hours.
2553064 Primary Facility 2017-01-18 Routine/Complete Inspection No Violations Noted The department of environmental protection conducted a routine inspection of the SWEPI LP Synnestvedt well site located in Osceola Township Tioga County on 1/13/2017. The weather was 30° partly sunny winds out of the north northwest 11 mph. Site was last inspected by company personnel on January 8, 2017 with the following comments "E&S inspection. Pad snow-covered." Erosion channels were noted on the access road below the pad entrance.

A strong odor was observed on the pad surface, similar to that of condensate. Frozen fluid was observed in the secondary containment surrounding the production tanks. The cut slope appeared stabilized however erosion control blanket is still visible in some areas. No violations were issued during this inspection. Please contact me with any questions. Pictures are included in the ESCGP file copy of the inspection report.

2627822 Primary Facility 2017-08-15 Site Restoration No Violations Noted I spoke with the landowner on site who said that the 3 producing natural gas wells on the pad are in 3 different formations (Marcellus, Burkett, Utica), and the natural gas is piped straight to UGI 's local market. EC readings with Oakton PCSTestr 35 (mS/cm): containment--0.37, ditch on side of pad--0.56.
2756658 Primary Facility 2018-07-26 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the SWEPI LP - Synnestvedt 878 well pad located in Osceola Twp, Tioga Co. on 7/26/2018 @1245 hours. The weather was 81 degrees, sunny, humidity 57% winds WSW 9mph.

The access road, well pad and surrounding controls were observed. The well pad measures approximately 655' x 265'. The sediment basin has been converted into an infiltration basin. Erosion channels were observed on upper portions of the access road. A small portion of the western fill slope is not stabilized by vegetation. Temporary BMPs were observed in rock-lined ditches. These temporary BMPs were noted in the restoration inspection dated 8/15/2017, in which the NOT was returned.

Due to conditions observed during the course of this inspection. It is recommended this NOT be returned. No violations are noted. I departed the site at 1400 hours.
