2442399 Primary Facility 2016-01-13 Routine/Complete Inspection No Violations Noted
2748478 Primary Facility 2018-07-03 Routine/Partial Inspection No Violations Noted SWEPI inspection log is in a mailbox near the production tanks, last inspected listed July 2, 2018.

I took electrical conductivity readings with an Oakton PCSTestr 35. A puddle on the pad gave a 0.67 mS/cm reading. JLP

2835228 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Bradford 481 Well pad in Sullivan Twp, Tioga County on 2/4/2019 at 1130 hours. The weather was partly cloudy and 50 degrees. Some slopes still showed some snow cover. The act 9 sign was shown in photo #1 and the E&S Plan was in the mailbox by the sign. A SWEPI Inspector was onsite currently performing a site inspection.

Five wells, the 481 1H & 3H and the 603 2H 4H & 6H, are all currently in production. The site remains stabilized and well vegetated. All PCSM practices remain in place and functional. All fluids and puddles field measured were within normal conductivity ranges. Containment structures were well maintained and no leaks were observed.

No violations noted today, I departed the site at 1215.
