2207668 Primary Facility 2013-09-26 Routine/Partial Inspection No Violations Noted On Sept 26, 2013 a routine partial inspection was conducted at the Sharretts 805 pad in Clymer Township, Tioga County. The last inspection conducted on behalf of the operator was on 9/26/2013. The 2H and 2HB wells have been drilled and a third well (23H) has recently been spudded. During this inspection the site was being re-graded in preparation for additional drilling in the coming weeks. The cut slope above the pad is in need of further stabilization. Some sediment transport to the rock-lined ditched was observed, but nothing has left the site. No violations were cited during this inspection.

2265508 Primary Facility 2014-04-25 Compliance Evaluation No Violations Noted On April 25, 2014 a compliance evaluation inspection was conducted at the Sharretts 805 pad in Clymer Township, Tioga County. The purpose of this inspection was to follow up on the progress of the operator in response to a Notice of Violation (307537) issued following a Feb. 12, 2014 inspection (2244667). The impacted soil has been excavated and disposed of properly. A summary report outlining the soil remediation and post-excavation was submitted by the operator (see permit file). No puddles of elevated conductivity were observed during today's inspection. The violations noted in 2/12/ 14 inspection report are resolved. No violation cited during this inspection.

2450856 Primary Facility 2016-02-18 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Sharretts 805 site located in Clymer Twp., Tioga Co. on 2/18/2016 @ 1230 hours. An adequate site ID sign was observed at the entrance to the access road. A copy of the E&S plan, well permits and PPC plan was located in the mailbox onsite. The onsite inspection log showed the site as last being inspected by company personnel on 2/17/2016 with the comments “site snow covered”. The receiving waters are listed as UNT to Jemison Creek HQ-CWF, MF.

Contracted personnel appeared to be doing maintenance work on production equipment during the inspeciton. All E&S BMPs are snow covered, but appeared to be functional. No violations were noted during this inspection. I departed the site @ 1330 hours. Please contact me with any questions.

2557333 Primary Facility 2017-02-01 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP Sharretts well site on 1/1/2017. The weather was 30?with heavy snow showers. All inspected secondary containment was full of snow. Erosion and sediment controls could not be observed due to snow. Site last inspected by company personnel on January 28 with "no changes" recorded in the comment section. No violations were noted during this inspection.
2581952 Primary Facility 2017-04-12 Routine/Complete Inspection No Violations Noted The DEP inspected the SWEPI LP Sharretts 805 well site in Clymer Twp. Tioga Co. on 4/12/2017 @ 1030 hours. The weather was sunny 56 degrees. The inspection was conducted to check the status of newly installed post construction Stormwater management BMV's. Some erosion and failure of ECB matting was noted. Please see attached pictures. The erosion does not appear to have been noted in the on-site E&S inspection log. Most newly restored areas appear to be stabilized by approximately 40% vegetative cover. No puddles were observed on the pad surface. No fluid was observed and secondary containment. Please Address any deficiencies noted, and contact me with any questions. I departed the site at 1130 hours.
2638337 Primary Facility 2017-09-19 Site Restoration No Violations Noted Restoration inspection conducted on 9/19/17. The Well Site Restoration Report (WSRR) and Notice of Termination (NOT) were received on 9/18/17. Interim restoration has been completed, post construction stormwater management BMPs installed, temporary E&S controls removed and the site permanently stabilized.
