2632833 Primary Facility 2017-09-06 Routine/Partial Inspection No Violations Noted Inspection to assess overall site conditions during drilling operations. I was accompanied by Zack Fidurko of PGE.

All drilling materials and equipment are within containment, with the exception of machinery being used for moving drilling pipe and other materials. A light sheen was observed on several puddles, and i recommended that it be removed. PGE indicated that it would be addressed.

No violations cited during this inspection.

2650199 Primary Facility 2017-10-20 Routine/Partial Inspection No Violations Noted Inspection to assess site conditions after well completion. A coil tubing rig is currently on site. Several small areas of staining were observed on the well pad. I informed the site foreman of the stained location and he indicated they would be cleaned up. Containment is still in place around the well head. The electrical conductance of the one puddle on the well pad was screened and was 540 uS/cm. No violations cited.
2811274 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Weather was 32° and cloudy at time of inspection. Met with PGE representative Dick Patrick on location. Well was shut during well construction to set 7 conductors and their cellars. Well had a 0-10k psi gauge on the production tubing at time of inspection. Tubing pressure was approximately 1000 psi. There was one annulus vent plumbed approximately 10 ft off the ground and was inaccessible to test with Altair 5x gas meter. 0% combustible gas was detected in the area surrounding he well at time of inspection. No violations were noted at time of inspection.
