2132828 Primary Facility 2013-01-18 Routine/Complete Inspection No Violations Noted
2141495 Primary Facility 2013-02-19 Routine/Complete Inspection No Violations Noted brine/flowback spill on the pad, ditches, and sed. basin. violations cited to the 117-21592 (3H) well
2301767 Primary Facility 2014-08-21 Routine/Complete Inspection No Violations Noted
2396276 Primary Facility 2015-07-29 Site Restoration No Violations Noted Interim restoration completed
2555839 Primary Facility 2017-01-25 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. No SWEPI representatives were present.

The pad has been partially restored. A Notice of Termination and Well Site Restoration Reports for the site were approved by the Department of Environmental Protection on July 29, 2015 (Inspection Nos. 2396253, 2396262, 2396274, 2396276). No evidence of accelerated erosion or sedimentation was observed during this inspection. No violations were cited.

2708439 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted Inspection to assess overall site conditions. No violations cited.
2856365 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Two wells are staged for production. The wells have well tags which say Shell Appalachia instead of Rockdale. The Department recommends replacing the well tags. Two GPUs are present, are labeled appropriately, and are in good condition overall. One steel double-walled produced water tank is present and in good condition overall. The tank is labeled appropriately. The pad surface has been partially restored. There is an observation water well located just south of the pad.

No E&S issues noted. No violations noted.
