Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2127230 Primary Facility 2013-01-03 Routine/Partial Inspection No Violations Noted A routine partial inspection of the Guillaume S 715 pad in Liberty Township was conducted on January 3, 2013. The well pad was snow-covered and no erosion issues were evident. The 1V well has been drilled and the 1H, 2H, 3H, 4H, 5H and 6H wells appear to be spudded. No violations were cited during this inspection.
2199533 Primary Facility 2013-08-29 Routine/Complete Inspection No Violations Noted
2204697 Primary Facility 2013-09-16 Drilling/Alteration No Violations Noted
2204864 Primary Facility 2013-09-16 Incident- Response to Accident or Event No Violations Noted
2222804 Primary Facility 2013-11-19 Routine/Complete Inspection No Violations Noted

2370483 Primary Facility 2015-05-12 Site Restoration No Violations Noted Arrived on site at 11:20, departed at 11:40. Weather conditions sunny and ~60°F. No SWEPI representatives were present at the time of inspection. This inspection was conducted in response to a Notice of Termination received by the Department.
2484334 Primary Facility 2016-05-09 Routine/Complete Inspection No Violations Noted
2557490 Primary Facility 2017-02-01 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions.

No evidence of accelerated erosion or sedimentation was observed. Vegetative cover was obscured due to snow. A Notice of Termination for the site's erosion and sediment control permit was approved on June 08, 2016.

No violations were cited during this inspection.

2718995 Primary Facility 2018-04-17 Routine/Complete Inspection No Violations Noted Weather was 32° cloudy and snowing at time of inspection. Rockdale representative Nathan Smelser was on location at time of inspection. Well had three annulus vents plumbed to surface. They were 2", 1 1/2"' and 1" pipe. The 5 1/2" x 9 5/8" annulus vent (2" pipe) had 12% methane detected on it at time of inspection. 0% combustible gas was detected in any other location at time of inspection. There was and analog pressure gauge attached to the tubing that read 0psi at time of inspection. Other digital gauges for remote monitoring were attached that did not have a readout display on them. No violations noted at time of inspection.
