2134435 Primary Facility 2013-01-24 Routine/Complete Inspection No Violations Noted Violations cited to the 6H well for a brine/flowback spill to the rock-lined ditch on the site.
2195009 Primary Facility 2013-06-19 Routine/Complete Inspection No Violations Noted
2198410 Primary Facility 2013-08-23 Follow-up Inspection No Violations Noted
2420423 Primary Facility 2015-10-20 Site Restoration No Violations Noted
2543442 Primary Facility 2016-12-08 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. No Southwestern representatives were present.

Overall vegetative cover on the site is greater than 70% and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The Erosion and Sediment Control General Permit (ESCGP) 1 for this site expired on October 14, 2015. Southwestern has categorized this site as one needing a 2-year restoration extension; however, the Department has not yet received a request for an extension of time. In order to remain in complaince with the Department's regulations regarding site restoration (Title 25 Pa. Code § 78a.65, Title 58 Pa. C.S. § 3216), Southwestern should obtain a new drilling permit, request an extension of time for site restoration, or restore the site. If the site is restored, a new ESCGP would be required.

No violations were cited during this inspection.

2668187 Primary Facility 2017-12-07 Routine/Complete Inspection No Violations Noted Inspection to assess overall site conditions. No issues identified and no violations cited.
2841015 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Site was snow covered at the time of inspection. Three wells are staged for production. There are no well tags on the well heads. The Department recommends labeling the well heads with well tags. One large white plastic produced water tank is present within steel secondary containment with plastic lining. The containment appears to be in good condition overall. The produced water tank is mislabeled as fresh water. The Department recommends replacing the label on the produced water tank.

No violations noted.
